Racial disparities exist in the accessibility, delivery, and quality of healthcare. Clinical images are central to plastic surgery, but choice literature is susceptible implicit biases. The objective this study was determine if published reflect racial demographic patients.A search for color photographs rendered graphics depicting human skin completed 6 surgery journals New England Journal Medicine Images each decade between 1992 2017. For article, were categorized as white or nonwhite based...
Background: The aesthetic aspects of the cleft lip nasal deformity have been appreciated for over a century, but functional implications remained largely underappreciated or misunderstood. This study describes frequency and severity obstructive symptoms among children with and/or palate, addressing hypotheses that age, type, are associated development symptoms. Methods: Children nonsyndromic palate comparison group unaffected born from 1997 to 2003 were identified through North Carolina...
Objective: In this study, the extent of racial diversity in images breast-related plastic surgery published literature was investigated to better understand disparities that exist breast surgery. Background: The lack skin color can perpetuate implicit bias and stereotypes. Implicit affect way patients are evaluated, diagnosed, treated. visual aspects make a imagery especially impactful on patient care outcomes. Methods: Published medical graphics depicting human were analyzed across 4 major...
Although many cleft teams have adopted nasoalveolar molding to improve nasal form, few comparative studies assessed the postoperative benefits of this treatment. Given that reported outcomes been contradictory and treatment involves considerable burden families, purpose study was assess objective subjective changes from at approximately 5 years age.All patients with complete unilateral lip palate who underwent primary cheiloplasty performed by a single surgeon over 7-year period were...
Implicit bias can lead to discrimination of certain populations within healthcare. Representation in medical literature is no exception and it hypothesized that images with lighter skin tone are more prevalent than darker tones craniofacial literature. Clinical photographs figure graphics from 5 journals were examined for pre-defined years. Annals Plastic Surgery, Aesthetic Surgery Journal, Journal Craniofacial Plastic, Reconstructive reviewed. All craniofacial-focused articles containing at...
We discuss the use of levothyroxine for excessive daytime sleepiness (EDS) and prolonged nocturnal sleep time in a euthyroid patient with narcolepsy. After failure first-line narcolepsy treatments, 48-year-old female began (25 mcg/day). After 12 weeks treatment, was evaluated improvement total subjective assessed by Epworth Sleepiness Scale (ESS).At baseline, ESS score 16 averaged h/day. weeks, 13 reported h/day.Levothyroxine improved EDS without cataplexy after side effects.
Objective Pain following primary palatoplasty can adversely affect nutritional and quality of life palatoplasty. The incorporation buccal flaps during cleft palate (CP) repair provides additional tissue to reduce the risk velopharyngeal insufficiency, though donor site may alter pain levels in postoperative period. This study sought compare patients undergoing CP with or without flaps. Design A single-institution, retrospective cohort study. Setting An academic children's hospital an...
Myelomeningoceles threaten newborns with central nervous system infectious risk. While some myelomeningoceles can be repaired fetally, limited donor tissue in makes covering a substantial defect challenging. This study evaluated the effectiveness of acellular dermal regeneration templates (ADRT) safely healing refractory myelomeningoceles.
Introduction: Maxillary distraction is often used to correct significant molar Class III malocclusion. We have introduced a modification of the traditional schedule utilizing larger interval distractions once week under sedation. hypothesize that this new still allows for successful advancement with limited relapse LeFort segment. The underlying principle greatest benefit maxillary I segment expand soft tissues palate and midface. Methods: modified consisted placement internal devices...
Introduction:Children with cleft lip and/or palate are at increased risk of obstructive sleep apnea. The prevalence apnea in this population is unknown. The purpose study was to assess the frequency symptoms and diagnosis population. We hypothesized that present more frequently children orofacial clefts than unaffected children, varies by sub-type. Methods: Children isolated without birth defects born 1997-2003 were identified through North Carolina Birth Defects Monitoring Program records,...
Background: Cranial vault reconstruction (CVR) is the gold standard in operative treatment of craniosynostosis. Full thickness osseous defects (FTOD) calvaria have been observed 5% to 15% patients after CVR, with higher rates cited fronto-orbital advancement (FOA) subset. Particulate bone graft (PBG) harvested manually has shown decrease FTOD FOA from 24% 5.5%. The authors used a modified technique using powered craniotome, hypothesis that would also improve outcomes. Methods: A...
Using longitudinal quantitative and qualitative data we examine the role that learning plays in school persistence dropout Ethiopia, India, Peru, Vietnam. Regression analysis shows a strong, significant association, with one standard deviation higher test scores associated 50% lower odds of dropping out between ages 8 12, similar association 12 15. Qualitative indicates direct relationship low dropout, children parents choosing to discontinue when they realize little is being learned....
Abstract Objectives To analyze relationships between Medicaid automatic enrollment for child Supplemental Security Income (SSI) recipients and health insurance coverage during transitions. Data Sources Study Setting Medical Expenditure Panel Study, 2000–2020 National Survey Children with Special Health Care Needs, 2001–2010. Design Leveraging variation in SSI‐Medicaid status across regions over time, we estimate a regression model to quantify associations coverage. We validate our findings...
INTRODUCTION: The purpose of this study was to evaluate long-term holistic outcomes for children with cleft lip and/or palate (CL/P) in North Carolina using a multidisciplinary, mixed-methods approach. This entailed cross-sectional evaluation population-based data, longitudinal clinical, and qualitative/semi-quantitative survey-based from the perspective patient/parent. Domains assessed include health, appearance, academic performance, behavior, school life social life, economics,...
Purpose: Breast surgery comprises a significant portion of plastic surgery. Given the high volume breast procedures, it is area investigation in literature. The aim this study was to use established photogrammetric analysis techniques evaluate visual representation racial diversity published literature and understand how compares general patient population. Methods: Leading journals field were selected for including Annals Plastic Surgery (APS), Aesthetic Journal (ASJ), Plastic,...
Midline clefts of the upper lip are rare, and it is therefore important that surgeons have access to a methodical approach for when these presentations encountered. We adapted principles anatomic subunit approximation unilateral cleft lip, repair midline clefts. The overt use landmarks define results in design inherently adjusts varying degrees can accommodate asymmetries. "measure twice, cut once" style an advantage new who seldom encounter this presentation. describe details surgical...
