As an idiopathic interstitial pneumonia, desquamative pneumonia (DIP) is uncommon form of lung disease and considered to be a smoking- or dust inhalation-related in the majority cases. Interstitial pneumonia is a common and major comorbidity affecting the prognosis of patients with systemic sclerosis (SSc). 背景.G-CSF産生肺癌は予後不良であることが知られている.症例.症例1は68歳,男性.血痰と右肩痛で受診し,精査の結果非小細胞肺癌cT3N0M1b(OSS)stage IVと診断し,全身化学療法を行うも奏効せず,3か月で永眠した.症例2は64歳,男性.健診胸部異常陰影で受診,EGFR遺伝子変異陰性の低分化肺腺癌cT3N0M0 stage IIBと診断し,腫瘍切除後に化学療法を行うも7か月で永眠した.いずれも白血球増多,血清G-CSF高値,免疫組織学的G-CSF陽性を認めた.結論.G-CSF産生肺癌は腫瘍の浸潤と増大が速く臨床上留意すべきであり,さらなる臨床症例の集積および基礎研究の発展による治療法確立が望まれる.
