Although associated with left heart pathologies, functional tricuspid regurgitation (FTR) is often untreated during surgery. Hence, owing to its degenerative character, reoperation needed, encompassing an impressive (25% 35%) mortality rate. Thus transcatheter approaches FTR are raising great interest. The authors evaluated the post-treatment effectiveness of edge-to-edge technique using percutaneous mitral valve repair device in ex vivo pulsatile model FTR. devices were implanted 11 porcine...
Abstract Background and Aims Surgical explantation of transcatheter heart valves (THVs) is rapidly increasing, but there are limited data on patients with THV-associated infective endocarditis (IE). This study aims to assess the outcomes undergoing THV explant for IE. Methods All who underwent between 2011 2022 from 44 sites in EXPLANT-TAVR registry were identified. Patients IE as reason compared those other mechanisms bioprosthetic valve dysfunction (BVD). Results A total 372 included....
Purpose: To analyze the results of isolated left subclavian artery (LSA) revascularization during thoracic endovascular aortic repair (TEVAR) using carotid-subclavian bypass (CSbp) or chimney grafts (CGs). Methods: A retrospective multicenter, observational study identified 73 patients (mean age 68±13 years, range 22–87; 56 men) with acute chronic lesions who underwent TEVAR LSA either CSbp (n=42) CGs (n=31) from January 2010 and February 2017. Primary endpoints were TEVAR-related mortality,...
The aim of this study was the analysis geometrical relationships between different structures constituting aortic root, with particular attention to interleaflet triangles, haemodynamic ventriculo-arterial junction and functional annulus in normal subjects.Sixteen formol-fixed human hearts roots were studied. root isolated, sectioned at midpoint non-coronary sinus, spread apart photographed by a high-resolution digital camera. After calibration picture resizing, software AutoCAD 2004 used...
We sought to evaluate whether the radial artery (RA) provides same clinical and angiographic results when proximally anastomosed aorta or left internal thoracic (ITA) as a composite Y-graft.From February 1999 December 2002, 512 patients underwent myocardial revascularization using RA, ITA and, required, saphenous vein. According surgeons' preference RA was [336 (65.6%), Ao-Cor group] Y-graft [176 (34.4%), group]. There significant prevalence of three-vessel disease (86.8 versus 73.2%, (P =...
The increasing incidence of calcific aortic valve disease necessitates the elaboration new strategies to retard progression pathology with an innovative solution. While diffusion transcatheter replacements (TAVRs) allows a mini-invasive approach substitution as alternative conventional surgical replacement (SAVR) in always larger patient population, TAVR implantation still has contraindications for young patients. In addition, it is liable undergo calcification consequent necessity...
We hereby present the case of a patient with severe aortic stenosis who underwent in her previous medical history mitral valve replacement mechanical (Omnicarbon 27), and progressively developed stenosis. This was judged inoperable then scheduled for CoreValve Revalving System implantation. Despite good positioning CoreValve, an acute, regurgitation soon after implantation as consequence impaired movement prosthesis leaflet. A condition cardiogenic shock quickly developed. function restored...
A 74-year-old woman was at the emergency department for acute chest pain, dyspnea and severe transient hypotension. History arterial hypertension external electrical cardioversion (EEC) persistent atrial fibrillation (AF) 8 days before admission. At that time echocardiography normal. The patient underwent coronary angiography with no evidence of significant arteries disease. a large multi-loculated mass occupying most left space obstructing ventricular inflow evident. There mild pericardial...
Bicuspid aortic valve (BAV) may be considered in the context of a phenotypic continuum configuration, ranging from severe forms unicuspid to rare quadricuspid valve. In this article, we report our results with BAV repair using surgical techniques tailored specific features continuum.Between September 2003 and May 2009, 31 patients (mean age 49.9+/-17.3 years; five female; 26 male) were prospectively enrolled for repair. The strategy was depending on anatomical structural characteristic...
Objective: Phasic blood flow dynamics and wall shear stress (WSS) have the potential to directly modulate endothelial responses, playing an important role in development of bypass graft occlusion. This study compares phasic velocity patterns WSS left internal thoracic artery (LITA) used as a composite Y-graft (27 patients, Y-group) single (24 S-group) on anterior descending (LAD) coronary artery. Methods: An intravascular Doppler-tipped guide wire was for postoperative analysis velocity.Flow...
Mycotic ascending aortic pseudoaneurysm (AAP) is an uncommon but surgically challenging problem with high morbidity and mortality rates. We describe endovascular repair of acute mycotic AAP in a high-risk patient. A 45-year old man, HIV serum positive, chronic hepatitis HBV HCV related, presented, after two sternotomies, fast growing 11 6 cm that was sealed Gore Exluder cuffs, inserted from the left axillary artery. Nine months control CT continued to show no endoleak shrinking AAP.
