背景．原発性肺癌の転移臓器としては脳，肺，骨，副腎などが多いといわれているが，眼球転移の報告は非常に稀である．組織型としては腺癌が多いと報告されており，眼症状を呈する場合の予後は短いことが知られている．今回我々は，肺腺癌の術後に眼球転移での再発を認めた症例を経験したため報告する．症例．72歳女性．X年11月に胸部異常影のために当科へ紹介受診した．精査の結果肺腺癌の診断となり，X＋1年2月に胸腔鏡下左下葉切除およびリンパ節郭清術を施行した．手術検体の病理診断はadenocarcinoma，pT1cN1M0 Stage IIBであった．慢性心不全を合併していたため術後補助化学療法は施行せずに外来で経過観察していたが，X＋1年8月の定期受診時に左眼球の突出および疼痛を認め，CTでは縦隔・鎖骨上窩リンパ節の多発腫大，左眼球腫瘍を認めた．縦隔リンパ節生検の結果，肺腺癌再発の診断となり，また他院にて摘出した左眼球腫瘍も同様の病理結果であった．結論．肺腺癌の術後再発で眼球転移を認めた症例を経験した．
背景．肺多形癌のEpidermal growth factor receptor（EGFR）遺伝子変異陽性率は約15～20%とされているが，common mutationが大多数でありexon 20挿入変異陽性例は稀少である．症例．62歳男性．血痰が出現し当科を初診した．CTで左肺下葉に腫瘤影を認めた．胸腔鏡下左肺下葉切除術を施行し肺多形癌pT3N0M0 Stage IIBの診断となった．その後腰痛，右大腿部痛を契機に第1腰椎・右大腿骨転子部，肺，左腎，左小脳への転移を認め再発と診断した．手術検体を用いて遺伝子変異検索を行いexon 20挿入変異が判明した．第1腰椎骨転移に緩和照射，右大腿骨に髄内釘留置を施行後にオシメルチニブ80 mg/日を導入した．開始1ヶ月後には肺病変の縮小と第1腰椎転移部位の溶骨性変化の改善を認めたが，3ヶ月後に腎転移の増大を認め病勢進行と判断した．結論．長期奏効は得られなかったが，EGFR exon 20挿入変異陽性肺多形癌という稀少癌種に対し右大腿骨の術後早期よりオシメルチニブによる治療導入を行い，一定の病勢制御が得られた1例を経験した．
