医療や介護の現場で日常的に発生する情報を集めた「リアルワールドデータ」は,近年,臨床研究で用いられる機会が多くなっている.本稿では,2017~2021年度に行われた大阪府後期高齢者歯科健診の約25万人分のデータと,国保データベースシステムの医療・介護データを基とした大規模コホート研究から,欠損歯列が欠損の拡大に与える影響について,筆者がこれまでに得た知見を紹介したい.さらに,臼歯部咬合支持と死亡との関連を評価した結果を踏まえ,Eichnerの分類,Cummerの分類,そして宮地の咬合三角といった咬合支持の観点から,欠損歯列が更なる歯の欠損,そして全身へ与える影響について考察する.
