症例は71歳,男性.近医で施行された腹部超音波検査にて肝S7に径50 mm大の腫瘍を指摘され,当院紹介となった.腫瘍マーカーはAFP,PIVKA-IIが高値であり,腹部CT,MRI,経動脈性血管造影CTで肝S7,S3に腫瘤を認めた.肝S3の腫瘤の造影効果は典型的ではなかったが,腫瘍マーカーの上昇およびB型肝炎ウイルスの既往感染の背景を考慮し,多発肝細胞癌と診断.肝S7亜区域切除術+肝S3部分切除術を施行した.病理組織学的検査で肝S7の腫瘍は数層からなる索状構造を形成しており,中分化型肝細胞癌と診断した.肝S3の腫瘍は,肝内胆管癌と細胆管細胞癌が混在しており,細胆管細胞癌成分を伴った肝内胆管癌と診断した.胆管癌であり部分切除では不十分と判断し,肝外側区域切除術およびリンパ節郭清を追加施行した.肝細胞癌と細胆管細胞癌成分を伴った肝内胆管癌の同時性重複癌の切除報告例は少なく,稀少性からも貴重な症例と思われたので,文献的考察を加え報告する.
