症例は75歳,女性.交通外傷で入院中に急性胆嚢炎を発症.外傷後で全身状態も改善しておらず,経皮経肝胆嚢ドレナージ(PTGBD)を選択した.その後,一度退院の希望が強く内視鏡的胆嚢ステント留置術(EGBS)を行い,PTGBDチューブを抜去し退院した.EGBSから3カ月後に,腹腔鏡下胆嚢摘出術目的に入院した.術前評価目的に腹部CT撮影したところ,胆嚢十二指腸瘻を認めた.腹部症状や全身状態に問題はなく,待機的に手術を行った.鏡視下で胆嚢と十二指腸の瘻孔部を丁寧に剥離して,ステントを抜去.瘻孔部は自動縫合器を用いて閉鎖することで容易に,かつ安全に閉鎖することができ,その後胆嚢を摘出した.術後合併症等なく経過している.EGBS後,遅発性に胆嚢十二指腸瘻を認めた本邦での報告例は稀である.適切に全身状態を評価した上で,腹腔鏡下にステント抜去し,自動縫合器で瘻孔閉鎖する方法は有用である可能性が考えられた.
