症例は75歳，女性．交通外傷で入院中に急性胆嚢炎を発症．外傷後で全身状態も改善しておらず，経皮経肝胆嚢ドレナージ（PTGBD）を選択した．その後，一度退院の希望が強く内視鏡的胆嚢ステント留置術（EGBS）を行い，PTGBDチューブを抜去し退院した．EGBSから3カ月後に，腹腔鏡下胆嚢摘出術目的に入院した．術前評価目的に腹部CT撮影したところ，胆嚢十二指腸瘻を認めた．腹部症状や全身状態に問題はなく，待機的に手術を行った．鏡視下で胆嚢と十二指腸の瘻孔部を丁寧に剥離して，ステントを抜去．瘻孔部は自動縫合器を用いて閉鎖することで容易に，かつ安全に閉鎖することができ，その後胆嚢を摘出した．術後合併症等なく経過している．EGBS後，遅発性に胆嚢十二指腸瘻を認めた本邦での報告例は稀である．適切に全身状態を評価した上で，腹腔鏡下にステント抜去し，自動縫合器で瘻孔閉鎖する方法は有用である可能性が考えられた．
