88歳,女性.後腹膜の濾胞性リンパ腫による水腎症のため,両側尿管ステント留置され,治療目的に当院転院された.入院中,発熱を認め,尿/血液培養にてAchromobacter xylosoxidansを検出した.感受性結果からtigecycline(TGC)を選択した.臨床所見は改善傾向にあったが,TGC投与開始14日目に凝固障害を認めた.経過と凝固検査からTGCによる凝固障害を考え,TGC中止のみで凝固異常は改善した.TGC使用時には凝固系のモニタリングが必要と考えられる.
