患者は41歳,女性.腹痛・嘔吐を主訴に当院を受診し,腹部CT検査にて気腫性胆嚢炎と診断された.臓器障害は伴わず中等症急性胆嚢炎に分類され,受診翌日に腹腔鏡下胆嚢摘出術を施行した.胆嚢壁は壊死しており,肝床部や大網との高度の癒着を認めたが,安全に腹腔鏡下手術を施行しえた.術後合併症なく第7病日に退院となった.気腫性胆嚢炎は中等症急性胆嚢炎の重篤な局所合併症の一つとされ,緊急手術の適応とされている.しかし,その手術手技の困難性から開腹術と腹腔鏡下手術とのいずれを選択すべきかについて一定の見解は得られていない.腹腔鏡下胆嚢摘出術にて安全に治療しえた気腫性胆嚢炎の1例を経験したので報告する.
