我々は先に血友病B患者20例の凝固第IX因子(FIX)遺伝子を解析し,18種類の点変異を報告した(血栓止血誌15 : 107-113, 2004).その中で未報告のA28P,Q50KとL300Pと,新たに検出したP193Lの計4種類の組換え変異体を作成し,これらが血友病Bを引き起こす原因であることを,発現実験を用いて確認した.変異FIX遺伝子プラスミドをHEK293細胞にトランスフェクション後の培養上清のFIX活性と抗原量は,野生型100%に対して,A28Pが12%と58%,Q50Kが4%と77%,P193Lが1%と4%,L300Pが<1%と5%,であった.細胞溶解液の抗原量は同様に,A28Pが79%,Q50Kが67%,P193Lが92%で細胞内合成を認めたが,L300Pは20%と他と比較し減少していた.4種類の未報告変異は,実験結果と各々の血漿表現型とほぼ一致しており,病因変異と同定した.

ハイレスポンダーインヒビター保有血友病Aおよび血友病B患者に起きた3回の重症出血に対し, 凝固因子製剤の大量投与でインヒビターを中和し, 引き続き持続投与によって充分な血漿凝固因子活性を維持し, 止血に成功した. 症例1は第VIII因子インヒビター保有血友病A患者. 38歳時に小脳出血で緊急入院した. 2. 1ベセスダ単位(BU)/ml の第VIII因子インヒビターに対し, 第VIII因子製剤5, 000Uのボーラス投与に続き, 3. 8U/kg/hrの持続投与を行った. 血漿第VIII因子活性は4日間0. 9~1.44U/ml を維持し血腫は縮小した. 40歳時には, 外傷性頭頂葉皮質出血をきたし, バイパス療法を行ったが, 新たに右硬膜下血腫を認めたためインヒビター中和療法を施行した. 10BU/ml 第VIII因子製剤12, 4~6U/kg/hrの持続投与を行った. 血漿第VIII因子活性は, 5日間0.54~2.04U/ml 症例2は4歳の第IX因子インヒビター保有血友病B患者. 右足背部の打撲により大きな皮下血腫を生じた. 連日のバイパス療法でも改善せず,...
