巨大な虫垂の軸捻転症の1例を経験したので報告する.症例は36歳,男性.既往歴に特記すべきことなし.平成16年2月26日早朝より下腹部痛が出現し,次第に増強したため同日夜当院を受診した.左上腹部から右下腹部にかけて腹部膨満あり,腹膜刺激症状は認めなかった. CT検査にて上部空腸と思われる腸管の著明な拡張を認めた.血液検査では白血球数の上昇を認めるのみであった.腹膜炎,絞扼性イレウスは否定的であったため,絶食,点滴による保存的治療を行った.しかし翌日になり腹部膨満がより著明となり,腹膜刺激症状も出現したため緊急開腹術を行った.拡張腸管は長さ28cmに及ぶ巨大な虫垂であり,虫垂が根部4cmを残して時計回りに360度捻転し,壊死に陥っていた.捻転を解除し,虫垂切除術を施行した.病理組織学的検査所見は虚血性変化であった.術後10日目に軽快退院した.
症例は10歳，男児．上腹部痛，嘔気を主訴に当科を受診した．以前より腹痛を繰り返し近医で胆嚢の位置異常を指摘されていた．腹部超音波検査，CT検査で正中に偏位し壁肥厚を伴った胆嚢を認めた．胆嚢結石は認めなかった．MRCP検査で胆嚢管の明らかな途絶像は認めなかったが胆嚢捻転症が否定できないため緊急腹腔鏡検査を施行した．胆嚢は著明に鬱血腫大し胆嚢管を中心に反時計方向に270度捻転していたため胆嚢捻転症と診断した．胆嚢は胆嚢管と胆嚢頚部の一部でのみ肝床部に付着しGrossI型の遊走胆嚢であった．捻転を解除した後，胆嚢摘出術を施行した．胆嚢捻転症は腹腔鏡手術のよい適応とされているが自験例では1）胆嚢管が肝床部に直接付着しておりCalot三角の展開が困難2）胆嚢管が長く，屈曲蛇行していたため三管合流部の確認が困難であった．症例によっては慎重な手術操作を必要とすると考えられた．
分岐異常を伴った右肝動脈（aberrant right hepatic artery；以下，ARHAと略記）が併存する膵頭部領域癌12例に対し，膵頭十二指腸切除術（pancreaticoduodenectomy；以下，PDと略記）を施行した．膵癌8，遠位胆管癌3，乳頭部癌1であった．ARHAの分類は，上腸間膜動脈より右肝動脈が分岐するA型，上腸間膜動脈より総肝動脈が分岐するB型，腹腔動脈より右および左肝動脈がそれぞれ独立分岐するC型，胃十二指腸動脈末梢より右肝動脈が分岐するD型の4型とし，各々6，1，3，2例であった．10例はARHAを温存し，2例はR0切除のためARHAを合併切除した．ARHAの再建1例，非再建1例で，後者は胆管炎を繰り返した．R0/非R0は各々7/5であった．非R0の要因は，膵周囲剥離面浸潤4，肝転移1で，全例が再発原病死した．ARHA症例にPDを施行する際，術前の周到な画像評価に加え，R0を行ううえで，ARHAの分岐形態も入念に検討する必要がある．
症例は69歳，男性．2008年1月，腹部膨満感・嘔吐を主訴に当科を受診した．腹部X線検査で小腸の鏡面像ならびに大腸の著明な拡張を認め，腸閉塞と診断した．腹部CT検査ではS状結腸壁の肥厚とその周囲脂肪濃度の上昇を認め，また回腸から下行結腸にいたる腸管拡張および腸液貯留を認めた．保存的治療では症状改善せず，また注腸検査によりS状結腸の完全閉塞を認めたことより腸穿孔が危惧されたため，入院翌日に緊急手術を施行した．S状結腸に腫瘤を触知し，S状結腸癌による腸閉塞の診断のもとD3郭清を行い，S状結腸切除術および下行結腸人工肛門造設術を施行した．切除標本では病変部の粘膜面に腫瘍性病変は認めず，憩室が散在しているのみであった．病理組織学的所見では，漿膜に陥入した憩室の粘膜上皮に癌腫を認め，漿膜へ全周に浸潤しており，憩室から発生した進行結腸癌と診断した．術後1カ月後に多発肝転移を認め，抗癌剤投与したが効果なく，4カ月後に死亡した．
