目的 探讨利多卡因-丙胺卡因乳膏(EMLA)致儿童高铁血红蛋白(MetHb)血症的临床特点及诊治方法。 方法 报道首都医科大学附属北京儿童医院2012年至2015年收治的EMLA致MetHb血症4例，并复习国内外相关文献，分析该病临床特点、发病机制及治疗方法。 结果 4例患儿年龄1～11个月，均有发绀及抽搐发作，其中呼吸心跳停止1例、肺出血1例、室性心动过速1例，予对症支持及亚甲蓝等治疗后均痊愈出院。检索中英文儿童EMLA致MetHb血症病例报道共11例，均经治疗痊愈。 结论 儿童EMLA致MetHb血症可有发绀、抽搐、意识障碍等表现，可危及生命，及时对症支持及合理使用亚甲蓝治疗预后良好。应用EMLA后有中毒表现的危重患儿，即使早期血MetHb分数升高不明显，仍可尝试亚甲蓝治疗，并应动态监测血MetHb分数。
