症例は68歳男性,前医にて食道癌に対して食道亜全摘術,後縦隔経路胃管再建術を施行された。発熱,咳嗽,呼吸困難感を自覚し,精査にて,胃管壊死による胃管気管支瘻と診断され加療目的にて当院に転院となった。胸部CTにて,右肺の広範な無気肺と両側胸水貯留を認めていた。気管支鏡検査では右上葉支分岐部末梢より気道内に壊死性物質の流入を認めた。全身状態も不安定なため緊急手術となった。手術は第5肋間側方切開にて開胸した。胃管は壊死しており右中間気管支幹と交通していた。瘻孔部位の組織は脆弱であり縫縮は困難と判断,広背筋弁を採取し,縫着した。閉胸後に頸部食道瘻および腸瘻を作成し手術を終了した。術後,筋弁の生着を優先するために分離換気 (左片肺換気,右はPEEP 5cmH2Oのみ) を行い,状態を見ながら徐々にPEEP圧を上昇させ,術後7日目に両肺換気を開始した。術後125日目には有茎空腸再建術を行い,現在,転院し引き続き加療を行っている。胃管気管支瘻の加療は非常に複雑かつ困難である。今回,診療科の連携により良好な経過を経た1例を経験したので報告する。

症例は43歳男性.原動機付自転車で走行中に自動車と衝突事故をおこし,当院救命救急科に搬送された.胸部CTにて左多発肋骨骨折,左血気胸,左下葉内に巨大肺囊胞を認めた.リザーバー付酸素マスク流量15 L/minで酸素投与も呼吸状態が保てなかったため挿管の上,人工呼吸器管理となった.入院2日目に気道内に出血を認め,経気道的に出血が疑われる左B6にトロンビン散布を行い一旦止血し得たが,翌日には再出血を認めた.再度トロンビン散布を試みたが止血できないため,当科紹介後に左下葉切除術を施行した.術後経過は良好であった.外傷性肺囊胞は胸部外傷に合併する肺内空洞性病変である.治療方針は保存的加療が基本だが,難治性出血,肺膿瘍,持続する肺漏を併発した症例に対しては手術適応となる.難治性出血を伴う本疾患は外科的治療を念頭に置いた診療が必要である.
