The use of propranolol is generally contraindicated in patients with congestive heart failure. ドック受診者の長期追跡調査を基に未破裂脳動脈瘤の保有率と破裂率を求め, その取り扱いについて考察した.対象は1993年7月〜2003年12月までにMRAを受けた9,641名のうち脳動脈瘤を認めた156例である.未破裂脳動脈瘤の保有率は1.6%と従来の報告より低めの値であり, 大きさは, 3〜7mmが78.7%を占めた.予防的処置を行わなかった121例の観察期間中6例(平均観察期間4.7年)に破裂を認め, Kaplan-Meier法による累積破裂率は, 1年で1.9% (3年: 4.6%, 5年: 8.5%)であった.また, 破裂6例中5例が7mm以下の脳動脈瘤であった.2003年のISUIAの研究によると, 破裂率は脳動脈瘤の大きさに関連し, 10mm未満でも7mmより大きいものは年間0.7%の確率で破れること, また6mm以下では年率約0.1%と破裂リスクは低いと述べている.しかし今回のわれわれの研究によれば, 日本人の脳動脈瘤は欧米人に比べ小さなものが多く, 小さなうちにより高い確率で破裂する可能性が示唆された.
