Sapphire is an attractive engineering material for the cover glass of smartphones and wristwatches because its high hardness resistance to corrosion, wear, heat. 背景.原発性悪性心膜中皮腫は非常に稀な疾患である.1剖検例と本邦における近年20年間の症例報告の集計を報告する.症例.58歳女性.呼吸困難を主訴に近医を受診し,胸部X線で縦隔腫瘍を疑われて当院へ紹介受診となった.胸部CTでは心基部に主要血管や主気管支を圧排する100×95 mmの巨大な縦隔腫瘍と軽度心嚢液貯留を認めた.エコーガイド下経胸壁針生検と画像所見から原発性悪性心膜中皮腫と診断した.Cisplatinとpemetrexedを投与し気道確保目的に緩和的放射線治療を開始したが,約2か月後に心タンポナーデ,腫瘍性DICを発症して永眠された.剖検では心嚢内に血腫と粘稠な血性心嚢液が認められ心タンポナーデの原因が判明した.また,上皮型の悪性心膜中皮腫であることがわかった.結論.原発性悪性心膜中皮腫の初発症状は心嚢液貯留を伴う心不全兆候が多い.心嚢液での診断率は低いため,早期診断のために外科的生検を要する.プラチナ製剤とpemetrexedによる化学療法が有効である.
