An 83-year-old man suffered from cerebral infarction of the right middle artery territory. In association with severe left hemiparesis and hemispatial neglect on side, he showed pusher phenomenon (PP), which made rehabilitation difficult. 【目的】Covered stentを用いて治療した巨大頚部仮性動脈瘤の2例を報告する.【症例1】72歳男性の末期癌患者.主訴は右頚部腫脹と嗄声であった.総頚動脈の蛇行が著しいため外科的に血管を露出して直接穿刺し,covered stentとbare stentを2重に留置して動脈瘤を閉塞させた.【症例2】71歳女性の進行癌患者.主訴は右頚部腫脹,嗄声及び呼吸困難であった.大腿動脈ルートでcovered stentを留置したが滑脱したため,bare stentで滑脱を修正した後,もう1本のcovered stentを用いて3重にステントを留置して動脈瘤を閉塞させた.【結論】Covered stent留置術は巨大頚部仮性動脈瘤に対して有効な治療法と考えられる.
