症例は75歳男性,4年前に肺腺癌(ⅡB期)に対し右下葉の摘出術を受け,術後カルボプラチン+ペメトレキセドを4コース施行した。その後,咳漱,労作時呼吸苦が出現増悪した。術前の胸部単純CTにて両下肺胸膜直下にスリガラス陰影を認めていたが,その後の胸部CTにて蜂巣肺,牽引性気管支拡張など両肺下葉末梢優位に陰影の増悪を認めた。特発性肺線維症を疑いプレドニゾロン(PSL),クラリスロマイシン(CAM),ピルフェニドンを投与するも改善に乏しく,今回PaO2 56mgHgと低酸素血症を認め,入院となった。問診にて30歳時より養鶏業に従事し,入院時は3,000羽の軍鶏を飼育していることがわかり,鳥関連過敏性肺炎を疑い,血清PDE(Pigeon dropping extracts)検査を施行した。PDE IgG 0.697,PDE IgA 0.445と高値を示し,慢性鳥飼病と診断した.画像からは特発性肺線維症との鑑別が困難であったが,問診とPDEが診断に有用であった。
