With the ongoing pandemic, it is critical to understand how natural immunity against SARS-CoV-2 and COVID-19 develops. We have identified that subjects with more severe disease mount a robust neutralizing antibody response spike protein.
Acute lung injury (ALI) and the more severe acute respiratory distress syndrome are common responses to a variety of infectious noninfectious insults. We used mouse model ALI induced by intratracheal administration sterile bacterial wall lipopolysaccharide (LPS) investigate changes in innate microbiota study microbial community reaction inflammation barrier dysfunction endotoxin insult. One group C57BL/6J mice received LPS via injection (n = 6), another water 7). Bronchoalveolar lavage (BAL)...
The Verigene tests for Gram-positive and Gram-negative organisms in blood culture the FilmArray identification panel were assessed their ability to identify pathogens from positive cultures. Both platforms correctly identified bacteria 92% of monomicrobial cultures analyzed, with times that significantly shorter than those subcultures.
Influenza vaccination aims to prevent infection by influenza virus and reduce associated morbidity mortality; however, vaccine effectiveness (VE) can be modest, especially for subtype A(H3N2). Low VE has been attributed mismatches between the circulating strains vaccine's elicitation of protective immunity in only a subset population. The low H3N2 2012-2013 season was egg-adaptive mutations that created antigenic mismatch actual strain (IVR-165) both intended (A/Victoria/361/2011)...
Abstract Objectives To evaluate the analytical and clinical performance of automated Elecsys anti–severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) antibody (Elecsys Ab) assay on Roche cobas e602 analyzer. With ongoing global disease 2019 (COVID-19) pandemic, widespread routine serologic testing SARS-CoV-2 remains a pressing need. better understand its epidemiologic spread to support policies aimed at curtailing further infections, reliable is crucial for providing insight into...
Abstract Background Mycobacterium scrofulaceum is a non-tuberculous mycobacteria (NTM) that has been recovered from numerous sources. The species rarely causes adult disease in immunocompetent hosts, but closely linked to pediatric cervical lymphadenitis as well the lungs, GI tract, and soft tissues of immunocompromised hosts. We report an M. pseudo-outbreak due sample cross contamination, which not previously reported. Methods University Chicago Medical Center tertiary care center...
This study describes an inexpensive and straightforward method for identifying bacteria by matrix-assisted laser desorption ionization-time of flight mass spectrometry (MALDI-TOF MS) directly from positive blood cultures using prewarmed agar plates. Different inoculation methods incubation times were evaluated to determine the optimal conditions. The two pelleted material culture bottles performed best. In particular, pellet streak correctly identified 94% Gram negatives following 4 h 98%...
ABSTRACT The Xpert Flu/RSV XC was compared to the FilmArray respiratory panel for detection of influenza (Flu) A, Flu B, and syncytial virus (RSV), using 128 nasopharyngeal swabs. Positive agreements were 100% A RSV 92.3% B. may be useful in clinical situations when extensive testing is not required serve an important role laboratories already performing broader testing.
Background Congenital toxoplasmosis is a treatable, preventable disease, but untreated causes death, prematurity, loss of sight, cognition and motor function, substantial costs worldwide. Objectives We asked whether high performance an Immunochromatographic-test (ICT) could enable accurate, rapid diagnosis/treatment, establishing new, improved care-paradigms at point-of-care clinical laboratory. Methods Data were obtained in 12 studies/analyses addressing: 1-feasibility/efficacy;...
Abstract As the Coronavirus 2019 (COVID-19) pandemic evolves, development of immunoassays to help determine exposure and potentially predict immunity has become a pressing priority. In this report we present performance EUROIMMUN enzyme-linked immunosorbent assay (ELISA) for semi-quantitative detection IgA IgG antibodies in serum plasma samples using recombinant S1 domain SARS-CoV-2 spike protein as antigen. Specimens from patients, with without COVID-19 infection, were tested at University...
Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) is currently causing a global pandemic. The antigen specificity and kinetics of the antibody response mounted against this novel virus are not understood in detail. Here, we report that subjects with more severe SARS-CoV-2 infection exhibit larger spike nucleocapsid protein epitope spreading to subdominant viral antigens, such as open reading frame 8 non-structural proteins. Subjects greater memory B cell spike, but protein....

This study assessed the accuracy of bacterial and yeast identification using VITEK MS, time to reporting isolates before after its implementation in routine clinical practice. Three hundred sixty-two bacteria yeast, consisting a variety American Type Culture Collection strains, were tested. Results compared with reference identifications from 2 system 16S rRNA sequence analysis. The MS provided an acceptable species level for 283 (78 %) isolates. Considering organisms which genus-level is...
Antimicrobial stewardship interventions utilizing real-time alerting through the electronic medical record enable timely implementation of bundle care (BOC) for patients with severe infections, such as candidemia. Automated candidemia using Epic module has been in place since July 2015 at our center. We sought to assess impact these alerts. All adult inpatients between April 1, 2011, and March 31, 2012 (pre-intervention), June 30, 2016, 2017 (post-intervention), were evaluated BOC adherence....

Results Sixty-eight...
We report a fatal case of Legionella feeleii endocarditis in post-lung transplant patient. The diagnosis was delayed, as routine microbiological testing nonrespiratory specimens does not account for extrapulmonary Legionella, and urine antigen only reliably detects pneumophila serogroup 1. This also illustrates the utility molecular sequencing blood culture-negative endocarditis.
Abstract Multiplex polymerase chain reaction (PCR) tests are useful for the rapid detection of pathogens, though diagnostic challenges may arise. We report 2 immunocompromised patients with Bordetella bronchiseptica respiratory infection misdiagnosed as pertussis using PCR, including discussion transmission, testing, clinical implications, and control considerations.
Abstract We evaluated the impact of Epic antimicrobial stewardship module (EAM) on number interventions, usage, and clinical outcomes. Use EAM allowed us to significantly increase ASP reviews interventions while maintaining a sustained utilization.
