Randomized clinical trials have provided estimates of the effectiveness BNT162b2 vaccine against symptomatic SARS-CoV-2 infection, but its effect on asymptomatic infections remains unclear. To estimate association vaccination with Pfizer-BioNTech and among health care workers. This was a single-center, retrospective cohort study conducted at tertiary medical center in Tel Aviv, Israel. Data were collected confirmed via polymerase chain reaction (PCR) tests workers undergoing regular screening...
Abstract In some immunocompromised patients with chronic severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection, considerable adaptive evolution occurs. Some substitutions found in infections are lineage-defining mutations variants of concern (VOCs), which has led to the hypothesis that VOCs emerged from infections. this study, we searched for drivers VOC-like emergence by consolidating sequencing results a set 27 Most reflected VOC mutations; however, subset associated...

Importance
Administration of a BNT162b2 booster dose (Pfizer-BioNTech) to fully vaccinated individuals aged 60 years and older was significantly associated with lower risk SARS-CoV-2 infection severe illness. Data are lacking on the effectiveness doses for younger health care workers.
Objective
To estimate association infections among workers who were previously 2-dose series BNT162b2.
Design, Setting, Participants
This prospective cohort study conducted at...
ObjectivesCarbapenem-resistant Gram-negative bacteria (CRGNB) pose a clinical challenge. We attempted to estimate the mortality burden of CRGNB among haematological cancer patients.
ABSTRACT Community-acquired lower respiratory tract infections (CA-LRTIs) treatment is largely empirical as microbiologic testing rarely performed. Here, we provide data of severe CA-LRTI cases requiring hospitalization. We aim to describe the distribution and susceptibility patterns pathogens causing CA-LRTI. analyzed samples from recently admitted patients with CA-LRTIs assess pathogen antibiotic patterns. divided into three groups: no prior healthcare exposure, diagnosed LRTI 48 7 days...
Multidrug-resistant organisms (MDROs) are important diabetic foot infection (DFI) pathogens. This study evaluated the impact of DFIs associated with MDRO pathogens (DFI-MDRO) on clinical outcomes.Adults admitted to Detroit Medical Center from January 2012 December 2015 culture-positive DFI were included. Associations between outcomes and DFI-MDRO (evaluated as a single group that included methicillin-resistant Staphylococcus aureus [MRSA], vancomycin-resistant enterococci, Enterobacteriaceae...
Abstract Background Recurrent Clostridioides difficile infection (rCDI) occurs frequently, and concomitant antibiotic (CA) during the initial episode for treatment of non-CDI is a major risk factor. We sought to address comparative efficacy fidaxomicin versus vancomycin in setting CA CDI episode. Methods conducted randomized, controlled, open-label trial at 2 hospitals Ann Arbor, Michigan. consecutively consented enrolled hospitalized patients ≥18 years old with diarrhea, positive test C....
During the recent pandemic, fact that clinical manifestation of COVID-19 may be indistinguishable from bacterial infection, as well concerns co-infection, have been associated with an increased use antibiotics. The objective this study was to assess effect targeted antibiotic stewardship programs (ASP) on antibiotics in designated departments and compare it equivalent same periods 2018 2019. Antibiotic consumption assessed days treatment (DOT) per 1000 patient (PDs). pandemic divided into...
Abstract Background Respiratory cultures are often obtained as part of a “pan-culture” in mechanically ventilated patients response to new fevers or leukocytosis, despite an absence clinical radiographic evidence suggestive pneumonia. Methods This was propensity score–stratified cohort study hospitalized adult between 2014 and 2019, with abnormal temperature serum white blood cell count (NATW), but without pneumonia, change ventilator requirements, documentation purulent secretions. Two...
Immunity is influenced by sleep and the circadian rhythm. Healthcare workers are predisposed to both insufficient disruption. This study aimed evaluate relationship between work characteristics antibody response mRNA SARS-CoV-2 vaccine BNT162b2. The authors' prospective cohort ("COVI3") evaluated effect of a third (booster) dose BNT162b2 vaccine. A subset participants provided information on anthropometric measures, sleep, stress including shift number hours per week. Blood samples for...
This study was performed to characterize the epidemiology, management, and outcomes of skin soft tissue infection (SSTI) colonization due carbapenem-resistant Enterobacteriaceae (CRE).Patients from Consortium on Resistance Against Carbapenem in Klebsiella Other (CRACKLE-1) December 24, 2011 October 1, 2014 with wound cultures positive for CRE were included study. Predictors surgical intervention analyzed. Molecular typing isolates using repetitive extragenic palindromic polymerase chain...
Abstract Background The polymicrobial nature of diabetic foot infection (DFI) and the emergence antimicrobial resistance have complicated DFI treatment. Current treatment guidelines for deep recommend coverage methicillin-resistant Staphylococcus aureus (MRSA) susceptible Enterobacteriaceae. This study aimed to describe epidemiology identify predictors associated with multidrug-resistant organisms (MDROs) pathogens resistant recommended (PRRT). Methods Adult patients admitted Detroit Medical...
Abstract In some immunocompromised patients with chronic SARS-CoV-2 infection, dramatic adaptive evolution occurs, substitutions reminiscent of those in variants concern (VOCs). Here, we searched for drivers VOC-like emergence by consolidating sequencing results from a set twenty-seven infections. Most this reflected lineage-defining VOC mutations, yet subset mutations associated successful global transmission was absent The these might dictate when infections can dramatically spread...
Abstract Clinical Enterobacteriacae isolates with a colistin minimum inhibitory concentration (MIC) ≥4 mg/L from United States hospital were screened for the mcr-1 gene using real-time polymerase chain reaction (RT-PCR) and confirmed by whole-genome sequencing. Four colistin-resistant Escherichia coli i solates contained . Two belonged to same sequence type (ST-632). All subjects had prior international travel antimicrobial exposure.
Abstract Background Microbiologic cure is a common outcome in pneumonia clinical trials, but its significance incompletely understood. Methods We conducted retrospective cohort study of adult patients hospitalized with bacterial who achieved cure. Rates recurrent and death were compared between persistent growth the index pathogen at time (microbiologic failure) those eradication cure). Results Among 441 patients, 237 experienced microbiologic 204 failure. Prevalences comorbidities,...
This study demonstrated a favorable short-term safety profile after third dose of the BNT162b2 vaccine among healthcare workers (HCWs). There were more frequent local reactions and less systemic compared to second dose. The HCWs who reported had higher prebooster titer anti-S1 antibodies those no reactions.
