はじめに:開腹既往歴がなく体表に原因のない小腸イレウスでは原因の検索が困難で治療方法が遅れ腸切除になることがある.方法: 1998年1月から2004年12月までの7年間に当院で手術した小腸イレウス295例のうち鼠径ヘルニアなど体表面から確認できる例を除いた開腹既往歴のない小腸イレウス36例の手術時期と腸切除の有無について検討した.結果:原因としては索状物によるもの12例,閉鎖孔ヘルニア10例,内ヘルニア5例,食餌性イレウス4例,腸重積4例,小腸捻転1例で食餌性を除き全例CTで診断できた. 14例に腸切除を行い, 22例は腸切除を行わずにイレウスを解除できた.腸切群では発症から手術までの時間が有意に長く,入院後24時間以内の手術例が有意に少なかった.考察:開腹既往歴のない小腸イレウスに対しては,速やかにCTを含めた検査を行い24時間以内に手術することにより腸切除を回避できる可能性が示唆された.

はじめに: 十二指腸潰瘍穿孔に対する保存的治療の適応基準を明らかにすることを目的とした.方法: 1997年4月から2004年3月までに入院治療した十二指腸潰瘍穿孔症例80例を対象に, 治療法により保存的治療群と手術治療群に分類し比較した. 保存的治療の適応は全身状態良好でUS, CTで腹水が少量で限局しているものとし, 1999年以降はこれに上部消化管造影検査で造影剤の漏出がないか, あっても漏出距離が2cm以下で限局しているものという条件を加え, この適応基準の妥当性を検討した.結果: 80例中46例に保存的治療を行い, 5例で腹腔内膿瘍に対する経皮的ドレナージを, 6例で手術に変更し, 結局40例 (87%) に完遂できた. 保存的治療完遂群が有意に若く (p<0.01), 入院日数は平均14日で手術治療群の平均22日と比べ有意に短かった (p<0.01). 上部消化管造影による適応基準導入後は保存的治療の完遂率は向上し, 腹腔内膿瘍の合併も低下した.考察: 今回の十二指腸潰瘍穿孔に対する保存的治療の適応基準は, その完遂率を向上させ,...
