Since "Helicobacter pylori (H. pylori) infection" was set as the indication in Japanese Society for Helicobacter Research (JSHR) Guidelines 2009, eradication treatment H. gastritis is covered under insurance since 2013 Japan, and number of has rapidly increased. Under such circumstances, JSHR made third revision to "Guidelines diagnosis first time 7 years. The Guideline Committee held 10 meetings. Articles published between establishment 2009 March 2016 were reviewed classified according...
The Japan Gastroenterological Endoscopy Society developed the Guideline for Endoscopic Diagnosis of Early Gastric Cancer based on scientific methods. diagnosis early gastric cancer has been acknowledged as a useful and highly precise examination, its use become increasingly more common in recent years. However, level evidence this field is low, it often necessary to determine recommendations expert consensus only. This clinical practice guideline consists following sections provide current...
ABSTRACT The catechin epigallocatechin gallate showed the strongest activity of six tea catechins tested against Helicobacter pylori (MIC for 50% strains tested, 8 μg/ml). It had bactericidal at pH 7 but not ≤5.0. In infected Mongolian gerbils, H. was eradicated in 10 to 36% catechin-treated animals, with significant decreases mucosal hemorrhage and erosion. Tea catechins, therefore, may have therapeutic effects on infection.
Fusobacterium varium may contribute to ulcerative colitis (UC). We conducted a double-blind placebo-controlled multicenter trial determine whether antibiotic combination therapy induces and/or maintains remission of active UC.Patients with chronic mild-to-severe relapsing UC were randomly assigned oral amoxicillin 1500 mg/day, tetracycline and metronidazole 750 vs. placebo, for 2 weeks, then followed up. The primary study end point was clinical response (Mayo score at 3 months after...
Objective Bleeding after endoscopic submucosal dissection (ESD) for early gastric cancer (EGC) is a frequent adverse event ESD. We aimed to develop and externally validate clinically useful prediction model (BEST-J score: ESD Trend from Japan) bleeding EGC. Design This retrospective study enrolled patients who underwent Patients in the derivation cohort (n=8291) were recruited 25 institutions, external validation (n=2029) eight institutions other areas. In cohort, weighted points assigned...
Abstract Background Helicobacter pylori ( H. )‐related diseases are responsible for a tremendous amount of morbidity and mortality in Japan. We estimated the prevalence infection by sex, birth year, geographic area among Japanese adults. Materials Methods This cross‐sectional study included 14,716 subjects aged 20 years or more who underwent health checkup between May 1997 March 2013 seven areas throughout Relevant information on demographics status was retrieved from electronic database....
Evaluation of Helicobacter pylori infection status (non-infection, past infection, current infection) has become important. This study aimed to determine the usefulness Kyoto classification gastritis for diagnosing H. by endoscopy. In this prospective study, 498 subjects were recruited. Seven well-experienced endoscopists blinded history eradication therapy performed examinations. Endoscopic findings assessed according gastritis: diffuse redness, regular arrangement collecting venules (RAC),...
Management of anticoagulants for patients undergoing polypectomy is still controversial. Cold snare (CSP) reported to cause less bleeding than hot (HSP).To compare outcomes between continuous administration (CA) with CSP (CA+CSP) and periprocedural heparin bridging (HB) HSP (HB+HSP) subcentimeter colorectal polyps.Multicenter, parallel, noninferiority randomized controlled trial. (University Hospital Medical Information Network Clinical Trials Registry: UMIN000019355).30 Japanese...
We performed a large, multicenter, randomized controlled trial to determine the efficacy and safety of early colonoscopy on outcomes patients with acute lower gastrointestinal bleeding (ALGIB).We an open-label study at 15 hospitals in Japan 170 ALGIB randomly assigned (1:1) groups that underwent (within 24 hours initial visit hospital) or elective (24-96 after hospital admission). The primary outcome was identification stigmata recent hemorrhage (SRH). Secondary were rebleeding within 30...
This study investigated the influence of urease-positive non-Helicobacter pylori bacteria on results a urea breath test (UBT) to evaluate diagnostic utility UBT using film-coated [(13)C]urea tablets. The was performed in 102 patients treated with proton pump inhibitor and antibiotics for eradication H. pylori. Urease-producing other than were isolated identified from oral cavity stomach. In 4/102 patients, gave false-positive results. These found be caused by presence Five bacterial species...
Background: The definition of Helicobacter pylori-negative gastric cancer depends on the accuracy diagnosis H. pylori infection. aim this study was to determine frequency and clarify relationships with histological atrophy, endoscopic serological atrophy. Methods: A total 240 early cancers were included in study. status infection determined from rapid urease test, 13C-urea breath culture, histopathological examination IgG antibodies. In cancer, atrophy intestinal metaplasia, assessed by...
Background and Aims: The results of a randomized controlled study meta-analysis have recently proved that Helicobacter pylori eradication has preventive effect against the development metachronous primary gastric cancer. However, cancer is sometimes detected after successful eradication. There lack about cancers in eradicated patients. To clarify characteristics H. eradication, we analyzed clinicopathological features these cancers. Methods: subjects were 18 early-stage specimens resected...
Prophylactic clipping has been widely used to prevent post-procedural bleeding in colon polypctomy. However, its efficiency not confirmed and there is no consensus on the usefulness of prophylactic clipping. The aim present study was evaluate preventive effect post-polypectomy bleeding.A multicenter randomized controlled conducted from January 2012 July 2013 Japan. Patients who had polyps <2 cm diameter were divided into a group non-clipping by cluster randomization. After endoscopic...
To prevent gastric cancer, a test-and-treat strategy for Helicobacter pylori has been proposed. This retrospective study assessed the clinical features, efficacy and safety of treatment H. infection in children adolescents.Questionnaires concerning features adolescents were sent to doctors 2013. It included questions on patient background, pylori-associated disease, first- second-line treatment, success or failure eradication, resistance antibiotics, occurrence adverse events. In 2014,...
Abstract Background The clinical significance of non‐ Helicobacter pylori (NHPH) is still unknown. There are many reports NHPH‐infected patients suffering from gastric diseases. Here, we investigated the polymerase chain reaction (PCR) positivity NHPH infection in disease who were negative for H. (Hp) by rapid urease test and pathological observation. Materials methods We collected 296 endoscopically obtained mucosal samples Hp‐negative diagnosed based on a pathology 17 hospitals Japan...
