Epstein-Barr病毒(EBV)属于疱疹病毒γ亚科,1964年由Epstein和Barr首次在Burkitt淋巴瘤细胞株中发现[1].EBV通过唾液传播,正常人群中感染率在90%以上,感染后终生携带EBV.EBV与多种疾病有关,有的仅有轻微症状,如同普通感冒;有的出现明显的全身性症状并呈自限性临床过程,如传染性单核细胞增多症(IM)[2];有的表现为长期反复发作的临床过程,并可进展为淋巴瘤,如慢性活动性EBV感染(CAEBV)[3];有的与肿瘤有明确关系,如Burkitt淋巴瘤、鼻型NK/T细胞淋巴瘤和霍奇金淋巴瘤等[1]。
1.病例简介:患者男,34岁.无明显诱因出现活动后乏力、气短,自觉头晕、头痛9个月入院.体检无明显异常.血常规示红细胞及白细胞减少,分片偶见原始细胞.骨髓穿刺示原始+幼稚淋巴细胞占59%.流式细胞学免疫分型CD33、CD7、CD11b、CD19、CD5、CD2、CD3阳性.染色体未见异常.临床诊断为急性淋巴细胞性白血病,双表型.于确诊后7个月进行骨髓同种异体干细胞移植.移植后给予环孢酰胺免疫抑制剂治疗,病情平稳.于移植后第55天检查发现患者右颈后可触及肿大淋巴结2枚,大小均约1.5 cm×1.0 cm.临床考虑:(1)白血病髓外复发;(2)病毒感染;(3)淋巴瘤.因患者移植术后2个月,尚未出无菌舱,不宜手术摘取淋巴结送检,因此于无菌舱内应用友谊式穿刺针[1]穿刺肿大淋巴结获取尽可能多标本,除制成1张细胞涂片外,剩余组织全部制成细胞块,常规切片,HE染色、免疫细胞化学染色及EB病毒原位杂交染色。
