Aim: This study aims to clarify the effectiveness of laparoscopic anatomical liver resections (ALRs) in surgical site infections (SSIs). Methods: We included 95 cases (44.0%) ALRs (LALRs) and 121 (56.0%) open (OALRs). Retrospective comparisons were performed between two groups. 喀血で緊急入院した33歳男性．気管支鏡検査で舌区入口部に凝血塊を認めたが，可視範囲内に出血源を認めず，CTでも気管支拡張症や慢性炎症像は認めなかった．入院中に再度喀血して低酸素血症に陥り，3D-CTで3mm径に拡張した気管支動脈を認めた．気管支動脈造影を施行したが，気管支動脈から肺静脈への血流が多いため塞栓術は合併症リスクが高いと判断し，左胸腔鏡補助下に両側気管支動脈結紮術を施行した．血痰は術後10日目に消失し，軽快退院した．術後5ヵ月，再発を認めていない．喀血に対する気管支動脈結紮術の報告は本邦で8例みられ，2例が術後1～14年で再発している．喀血に対する気管支動脈結紮術は有用な選択肢であるが，再発に対する術後長期の観察が必要である．

症例は64歳の男性で,黄疸を主訴に近医を受診し精査にて膵頭部癌と診断され紹介受診した.術前検査で,PT-INR 2.52,APTT 63.1秒と延長,第V因子活性5%と低下,第V因子インヒビター陰性であり,第V因子欠乏症を合併した膵頭部癌と診断した.術前に新鮮凍結血漿(fresh frozen plasma;以下,FFPと略記)を輸血し凝固機能の改善を確認し,亜全胃温存膵頭十二指腸切除術(subtotal stomach-preserving pancreaticoduodenectomy;SSPPD)を施行した.周術期は凝固機能を測定しFFP輸血を術後4病日まで投与し後出血を起こすことなく術後第11病日に退院した.本症例のような閉塞性黄疸症例では血液凝固異常を来していることは少なくないが,本疾患を含めた血液凝固異常の疾患についても注意が必要と考える.

症例は56歳,男性.左頸部腫瘤を主訴に受診した.既往歴に慢性B型肝炎および肝細胞癌があり,半年前近医で肝動脈塞栓療法を施行された.診察上,左鎖骨上窩に小指頭大の弾性硬の腫瘤を数個触知した.局所麻酔下にリンパ節を1個切除し肝細胞癌の転移と診断されたため,全身麻酔下に左頸部郭清を行った.術後他院にて残存肝腫瘍に対してラジオ波焼灼療法を行った.術後1年目に右頸部腫瘤を触知し,CTおよびPET検査にて右鎖骨上窩から縦隔と胸骨傍リンパ節再発,肝S6は残存腫瘍と診断され,右頸部リンパ節および右開胸下縦隔,胸骨傍リンパ節郭清を行った.病理診断は,肝細胞癌リンパ行性転移と診断された.その後肝動脈化学塞栓療法およびソラフェニブによる化学療法を行ったが,原発巣の増大を認め術後1年7カ月後に肝不全で死亡した.肝内病変がコントロールされていない症例のリンパ節郭清および肝外病変の手術は慎重に選択すべきと思われる.
In our former papers, the mitigation of across-wind responses high-rise building was confirmed. Next, we discuss that along-wind oscillations have to be considered for with hysteretic damper. When consider damper, it is an important point external wind forces mean component continuously. So need influence and fluctuate components on this paper, show damper compare them oscillations.
