79歳の男性。1989年の右腋窩に結節が出現した。近医にて液体窒素療法を繰り返し受けるうち, 同部は潰瘍化した。その後潰瘍は徐々に拡大し, 2002年7月頃より潰瘍は急速に拡大してきたため同年10月1日当科を受診した。初診時右腋窩に6×5cmの蚕食性の潰瘍を認め, 一部に白苔を伴っていた。潰瘍周辺には衛星状に黒色の小結節を認めた。生検にて基底細胞癌と診断し, 10月24日, 衛星病変を含め辺縁より2cm離し, 底部は肋骨骨膜上にて一塊に切除した。また術中迅速病理検査にて腋窩リンパ節に転移が認められたため, 同時に腋窩リンパ節を可及的に摘出, 創部は分層植皮により被覆した。術後4カ月後に右腋窩部に局所再発が認められたため追加切除を施行。経過観察中であるが追加切除後1年2カ月間, 再発は認められていない。
