当科で経験した乳房外Paget病100例についてまとめ，1992年に報告した内容と比較検討した。本疾患は70代男性に多くみられ，近年は増加傾向にあった。大部分の症例は表皮内癌で生命予後は良好であったが，浸潤癌症例では高率にリンパ節転移を来しており，予後不良であった。当科では従来の方針を見直し，皮膚側切除範囲を5cmから3cmにまで縮小し，それまで全例に行ってきた所属リンパ節郭清の適応を浸潤癌の症例に限定してきた。しかし，原発巣の局所再発は認めず，生命予後の悪化もなく，取扱い規約を支持する結果であった。今後高齢化社会を迎え，症例の増加が見込まれる。切除範囲のさらなる縮小およびセンチネルリンパ節生検などの手技を用いたリンパ節郭清適応症例の見極めが必要になると思われた。

79歳の男性。1989年の右腋窩に結節が出現した。近医にて液体窒素療法を繰り返し受けるうち, 同部は潰瘍化した。その後潰瘍は徐々に拡大し, 2002年7月頃より潰瘍は急速に拡大してきたため同年10月1日当科を受診した。初診時右腋窩に6×5cmの蚕食性の潰瘍を認め, 一部に白苔を伴っていた。潰瘍周辺には衛星状に黒色の小結節を認めた。生検にて基底細胞癌と診断し, 10月24日, 衛星病変を含め辺縁より2cm離し, 底部は肋骨骨膜上にて一塊に切除した。また術中迅速病理検査にて腋窩リンパ節に転移が認められたため, 同時に腋窩リンパ節を可及的に摘出, 創部は分層植皮により被覆した。術後4カ月後に右腋窩部に局所再発が認められたため追加切除を施行。経過観察中であるが追加切除後1年2カ月間, 再発は認められていない。

Abstract A 22‐year‐old Japanese woman with Koebner epidermolysis bullosa simplex presented severe palmo‐plantar hyperkeratosis. 82歳，女性の鼻部から右内眼角に及ぶmorpheaformの基底細胞癌（BCC）を報告した。病巣部切除後，全層植皮術を行った。術後2年4ヵ月以後は来院がなく，97歳，14年半ぶりの再診時には，再発腫瘍が眼瞼ならびに眼窩に浸潤していた。CT画像上，眼窩脂肪織や周囲骨組織への浸潤所見を認めた。軽度認知症を伴う超高齢者故に，眼窩内容除去術を行わずに，姑息的治療で経過を追っているが，最近のCT，MRI画像上では，脳硬膜，右海綿静脈洞，斜台，ほかへの浸潤所見を認めている。 眼周囲のBCCでは，再発が少なからず起こるので，密な間隔で，長期的な追跡が必要である。
