Background. Animal and limited human studies have raised concerns as to the safety of in utero exposure mycophenolate mofetil (MMF) sirolimus (SRL) transplant recipients. This study examined outcomes pregnancies with MMF or SRL from 30 female recipients (39 pregnancies) who reported National Transplantation Pregnancy Registry. Methods. Data were collected via questionnaires, phone interviews medical records. Results. There 18 kidney reporting 26 MMF: 15 livebirths (LB), 11 spontaneous...
Heart transplant recipients (HTRs) during pregnancy are at greater risk for maternal and obstetrical complications hypertensive disease of exacerbates these risks. The impact preeclampsia on HTRs is unknown. authors describe characteristics who developed the effect graft outcomes. This a retrospective group study adult with subsequent outcomes ≥20 weeks' gestation enrolled in Transplant Pregnancy Registry International between 1986 2022. primary outcome was loss within 2 years from delivery....
The purpose of this study was to analyze pregnancy outcomes in female lung transplant recipients. Data were collected from the National Transplantation Pregnancy Registry via questionnaires, interviews, and hospital records. Twenty-one recipients reported 30 pregnancies with 32 (1 triplet pregnancy). Outcomes included 18 live births, 5 therapeutic abortions, 9 spontaneous abortions. No stillbirths or ectopic reported. Mean (SD) interval conception 3.6 (3.3) years (range, 0.1–11.3 years)....
CONTEXT-In women, exposure to mycophenolic acid products during pregnancy results in an increase both miscarriages and birth defects the live born. OBJECTIVE-To describe outcomes of pregnancies fathered by transplant recipients who were being maintained on at estimated time conception compare these with general population. METHODS- Data collected National Transplantation Pregnancy Registry via questionnaires, telephone interviews, medical records. RESULTS -One hundred fifty-two male 205 (208...
Solid organ transplantation (SOT) offers people with end-stage disease an increased quality of life, which includes the return fertility and potential for pregnancy. Although number pregnancies has increased, definitive recommendations have been lacking. To address reproductive health in SOT recipients, American Society Transplantation Women’s Health Community Practice held a virtual Controversies Conference subject matter experts gathered to discuss topics contraception, immunosuppression,...
Rates of cesarean delivery (CD) are increased among transplant recipients. There is a need to define the indications for CD and associated outcomes recipients determine safest mode obstetric delivery.
Impaired fertility is common among patients with chronic organ failure, including end-stage renal disease (ESRD). Women of childbearing age undergoing transplantation may experience rapid return fertility. Pregnancy posttransplant presents numerous risks for the patient, fetus, and allograft. Maternal include hypertension preeclampsia. Allograft acute rejection failure organ, fetal miscarriage, birth defects from immunosuppressants, premature delivery, low weight. Belatacept, a selective T...
Thousands of women with organ transplantation have undergone successful pregnancies, however little is known about how the profound immunologic changes associated pregnancy might influence tolerance or rejection allograft. Pregnant a solid transplant are complex chimeras multiple foreign cell populations from donor organ, fetus, and mother pregnant woman. We consider impact chimerism pregnancy-associated on allograft both during postpartum period. Mechanisms likely dynamic affected by influx...
OBJECTIVE: To evaluate the clinical and laboratory characteristics in pregnancy that differentiate preeclampsia from acute renal allograft rejection to investigate maternal, neonatal, graft sequelae of these diagnoses. METHODS: We conducted a retrospective case-controlled registry study data abstracted Transplant Pregnancy Registry International deliveries between 1968 2019. All adult kidney transplant recipients with singleton pregnancies at least 20 weeks gestation were included. Acute was...
Background. Posttransplant fertility returns quickly, and female recipients of child-bearing age may conceive while on immunosuppression. However, pregnancy after transplantation confers risks to the recipient, transplant, fetus, including gestational hypertension, preeclampsia, diabetes, transplant dysfunction, preterm labor, low birthweight infants. Additionally, mycophenolic acid (MPA) products are teratogenic. Literature evidence regarding belatacept, a selective T-cell costimulation...
OBJECTIVE: To define risk factors, obstetric morbidity, and allograft outcomes associated with an unintended pregnancy after kidney transplant. METHODS: This is a retrospective cohort study of pregnancies in women transplantation enrolled the Transplant Pregnancy Registry International births between 1967 2019, cohorts categorized as intended pregnancies. The primary outcome was acute rejection during or by 6 weeks postpartum. Secondary included loss, severe maternal neonatal composite...
