切除不能局所進行食道癌に対する化学放射線療法は,長期生存が見込める治療であるが,穿孔や穿通などの致死的合併症発症の可能性がある.今回,大動脈浸潤食道癌に対する化学放射線療法中に発症した大動脈食道瘻に対して,大動脈ステントグラフトで止血した症例を経験した.症例は67歳の男性で,2011年8月に嚥下困難で発症し,大動脈浸潤食道癌と診断された.2011年9月から化学放射線療法を施行し,部分奏効した.2012年1月に吐血によるショック状態となり,大動脈食道瘻と診断し,大動脈ステントグラフトで止血した.食道狭窄症状を認めたため,2012年4月食道カバードステントを挿入した.経口摂取が可能になり,自宅退院した.2012年5月,原疾患が進行し死亡した.大動脈食道瘻は致死的であるが,大動脈ステントグラフトによって急性期救命され,経口摂取が可能になったため,低侵襲で効果的な姑息的治療であると思われた.

症例は47歳,女性,検診の腹部エコーで下腹部腫瘍を指摘された.MRIで約11cmの骨盤内腫瘤を認め,漿膜下子宮筋腫の診断で本人希望で経過観察となったが頻尿が出現し,初診から1年7カ月後に手術となった.開腹所見は回腸間膜基部の腫瘍であった.腫瘍を核出して間膜を閉鎖し,手術を終了とした.術後の病理組織診断はaggresseive angiomyxomaであった.本疾患は若い女性の外陰部から骨盤に好発するが,それ以外の部位に発生すことは稀である.再発の多い腫瘍でもあり,今後も慎重な経過観察が必要と思われた.

症例は33歳男性.近医でS状結腸憩室炎の診断で入院加療した.退院後4日で腹痛が再燃し当院受診.外来で内服抗生剤を処方され経過観察となったが腹痛は持続し排尿時痛,気尿も認めたため外科紹介,緊急入院となった.尿検査で膿尿を認め,腹部CTで膀胱内に空気貯留を認めた.骨盤単純MRIで瘻孔が描出されS状結腸膀胱瘻と診断された.膀胱鏡で膀胱三角部付近に強い炎症性変化を認め瘻孔や尿管口ははっきりしなかったため一期的手術とせず,横行結腸人工肛門を造設し,6カ月後に膀胱鏡を併用して腹腔鏡下S状結腸切除,膀胱瘻孔部閉鎖術を行った.更に1カ月後人工肛門を閉鎖し,術後4カ月で再燃などを認めない.
