症例は73歳女性. 69歳のとき腎硬化症による慢性腎不全のため, 腹膜透析を導入した. 73歳のときランソプラゾールの内服を開始し, 3か月後, 3回目の腹膜炎で入院となった. セファゾリンとセフタジジムを経静脈, その後腹腔内投与に切り替え計2週間投与し, 軽快退院した. 入院中に軟便であったが便Clostridium difficile抗原は陰性であった. 退院後から水様～泥状便が持続し, 外来にて便Clostridium difficile抗原陽性となり, 偽膜性腸炎の診断でメトロニダゾールを14日間内服した. その後も下痢は持続し, 食思不振, 炎症反応高値を認めたため, 精査加療目的に当科入院となった. 絶食・中心静脈栄養を開始した. 大腸内視鏡検査を施行し, 肉眼で上行結腸に炎症瘢痕を認めた以外は異常所見を認めなかったが, 直腸, 盲腸, および結腸の粘膜生検で粘膜表層上皮下にリンパ球浸潤を認め, Masson Trichrome染色で特徴的なcollagen bandの形成を認めたため, collagenous colitis (CC) と診断した....
