副甲状腺機能亢進症の唯一の治療は病的副甲状腺の摘出である。近年,検診などで偶発的に発見される原発性副甲状腺機能亢進症や,透析治療の合併症である二次性副甲状腺機能亢進症が増加傾向にある。今回われわれは当科で過去2年間に経験した副甲状腺機能亢進症手術症例29例に対して病的副甲状腺の局在につき頸部超音波検査やcomputed tomography (CT),technetium 99m 2-methoxyisobutyl-isonitrile scintigraphy (MIBIシンチ) など術前画像診断の正診率や,術後合併症として近年問題となっているhungry bone syndromeの予測因子等につき検討した。その結果,各術前画像診断の有用性が確認され特に頸部超音波検査とMIBIシンチを組み合わせることにより90%以上の高い局在診断率が得られた。また,hungry syndrome合併の予測因子として今回の術前評価項目 (血清アルカリフォスファターゼ,血清尿素窒素,年齢,術前後でのインタクトPHT減少率) の中では血清アルカリフォスファターゼの高値のみが予測因子となりうる可能性が示唆された。
