In a retrospective analysis on 323 orthotopic liver transplant procedures performed between July 1984 and October 1991 the incidence of two forms primary dysfunction (PDF) liver: nonfunction (PNF), initial poor function (IPF) were studied. The PDF was 22% (73/323) with 6% PNF (20/323) 16% IPF (53/323), while 78% (250/323) had immediate (IF). Occurrence both resulted in higher graft failure rate (P < 0.001), retransplantation patient mortality 0.003) within first three months after OLTx....
A national conference on organ donation after cardiac death (DCD) was convened to expand the practice of DCD in continuum quality end-of-life care. This affirmed ethical propriety as not violating dead donor rule. Further, by new developments previously reported, resolved controversy regarding period circulatory cessation that determines and allows administration pre-recovery pharmacologic agents, it established conditions eligibility, presented current data successful transplantation organs...
Methods of accurately assessing the suitability donor livers prior to transplantation are required if incidence primary nonfunction (PNF) is be reduced. This study evaluated ability liver biopsies predict development after transplantation. From June 1987 until May 1990, 170 transplants were performed in 147 patients. A total 124 obtained and divided into three groups. Group 1 (n=77) revascularization liver, group 2 (n=19) hepatectomy but examined only implantation, 3 (n=28) as 2,...
Campath‐1H, an anti‐CD52 monoclonal antibody, was used as induction therapy (40 mg i.v. total dose) in 29 primary human renal transplants, and the patients were maintained on rapamycin monotherapy (levels 8–15 ng/mL) post‐transplant. Campath‐1H profoundly depletes lymphocytes long‐term more transiently B cells monocytes. All are alive well at 3–29 months of follow up. One graft lost because rejection. There have been no systemic infections malignancies. Eight experienced rejection, which...
This study evaluates the long-term outcomes, biliary complication rates, and risk factors for complications after liver transplantation from "donation cardiac death" (DCD) donors.Recent enthusiasm toward increased use of DCD donors' livers is mitigated by high rates. Predictive development remain incompletely defined. We performed a retrospective review 1157 brain (DBD) 87 transplants between January 1, 1993, December 31, 2008. Patient graft survivals rates within first year were compared DBD...
The American Society of Transplant Surgeons (ASTS) champions efforts to increase organ donation. Controlled donation after cardiac death (DCD) offers the family and patient with a hopeless prognosis option donate when brain criteria will not be met. Although DCD is increasing, this endeavor still in midst development. protocols, recovery techniques acceptance vary among procurement organizations transplant centers. Growing enthusiasm for has been tempered by decreased yield transplantable...
Simultaneous pancreas-kidney transplantation (SPK) is a procedure which frees the diabetic patient with end-stage nephropathy from dialysis and daily insulin injections. The purpose of this study to report long-term outcomes procedure, describe surgical medical complications. The analysis includes 1000 consecutive SPKs performed between 1985 2007. Bladder drainage was used in 390 patients enteric 610 patients. In 362 patients, SPK before initiation dialysis.Patient survival at 1, 10, 20 years...
To compare conventional low-temperature storage of transplant donor livers [static cold (SCS)] with the organs at physiological body temperature [normothermic machine perfusion (NMP)].
From December 1985 to October 1997, 500 simultaneous pancreas-kidney transplants (SPKs) were performed at the University of Wisconsin. Bladder drainage (BD) was used in 388 and enteric (ED) 112. All pancreas preserved UW solution.Patient survival 1, 5, 10 years 96.4%, 88.6%, 76.3%; kidney function, 80.3%, 66.6%; 87.5%, 78.1%, 67.2%. Thrombosis occurred three four (0.6% 0.8%) primary nonfunction one (0.2%). There a 4.2% acute tubular necrosis rate for kidney. Conversion from BD ED required...
The role of humoral alloreactivity in ABO-compatible liver transplantation remains unclear. To understand the significance donor-specific HLA alloantibodies (DSA) rejection, we applied currently used strategy for detection antibody-mediated rejection other solid allografts. For this purpose reviewed data on 43 recipients ABO identical/compatible donor livers who had indication biopsy stained complement element C4d and contemporaneous circulating DSA determination. Seventeen (40%) patients...
Abstract The prevalence of the metabolic syndrome with attendant morbid obesity continues to increase nationwide. A concomitant in non‐alcoholic steatohepatitis ( NASH ) and associated end‐stage liver disease requiring transplantation is expected parallel this trend. Between J anuary 1, 1997 D ecember 31, 2008, our center performed 813 solitary adult deceased‐donor transplants. Patients were divided into groups based on W orld H ealth O rganization I nternational C lassification obesity....
Renal transplant outcomes result from a combination of factors. Traditionally, donor factors were summarized by classifying kidneys as extended criteria or standard criteria. In 2014, the nomenclature changed to describe with kidney profile index (KDPI). We aim evaluate relationship between KDPI and delayed graft function (DGF), impact on for both after cardiac death (DCD) brain (DBD).An IRB-approved single-center retrospective chart review was performed January 1999 July 2013. The patients...
We report on the efficacy and safety of FK506 (tacrolimus) compared with a cyclosporine (CsA)-based immunosuppressive regimen after 1 year treatment in pediatric liver allograft recipients (< 12 years) participating multicenter U.S. randomized trial. Patients received either or CsA as primary immunosuppression following first ABO-compatible transplant. Intravenous was initiated at 0.1 mg/kg per day, followed by oral beginning 0.3 day. The dose adjusted to maintain plasma trough levels...
Donation after cardiac death is a method by which severely neurologically injured patients not fulfilling brain-death criteria can donate organs. To develop an evaluation tool that be used to predict if patient suitable candidate for donation death. The University of Wisconsin After Cardiac Death Evaluation Tool assigns numeric values observable clinical parameters yield overall predictive score suitability death. This typically utilized in critical care unit evaluate with severe neurological...
Objective To compare the outcome of simultaneous pancreas-kidney transplantation (SPK) and living related donor renal (LRD) in patients with diabetes. Summary Background Data It remains unanswered whether diabetic end-stage failure are better served by LRD or SPK. Methods Using a longitudinal database, data from all receiving cadaveric transplants SPKs January 1986 through 1996 were analyzed. Patient graft survival, early function, cause patient loss compared for 43 HLA-identical LRDs, 87...

The urological complications of 210 patients who underwent simultaneous pancreas-kidney (SPK) transplantation over a 7-year period were reviewed.Worldwide, bladder drainage has become the accepted method exocrine after pancreas transplantation. With increasing use drainage, surgical post-transplant have shifted from intra-abdominal to complications. Two hundred ten diabetic received SPK transplants with drainage. A retrospective review was conducted analyze incidence, type, and management...
With refinements in surgical techniques and increased clinical experience, there has been a resurgence of interest vascularized pancreas transplantation. From December 1986 to April 1988, 30 whole-organ transplants with pancreatico duodenocystostomy were performed simultaneously renal The recipient population consisted 20 men ten women, mean age 34.7 years (range 25–53 years). duration insulin-dependent diabetes mellitus (IDDM) was 22.6 10–37 preservation time 8.7 hours 3–19 AH patients...
