Bacillus cereus is an uncommon nosocomial bacteria, typically dismissed as a contaminant. This case unique scenario in which B. bacteremia persisted despite appropriate treatment. Further investigation revealed the presence of right atrial thrombus believed to harbor biofilm responsible for sustained bacteremia. Clearance using AngioVac system (AngioDynamics, Inc., Latham, NY) led resolution blood cultures, and subsequently, patient was discharged with six-week course intravenous (IV) antibiotics.
