要旨:症例は神経線維腫症1型の25歳女性.左頸部腫脹と呼吸困難が3日間の経過で進行し,気道閉塞を来したため当院へ搬送された.CT上左頸部皮下軟部組織全体にわたる血腫を認め,正中偏位を来していた.血管撮影では左頸部内頸動脈の紡錘状の動脈瘤とともに左顔面動脈から血管外漏出を認めた.後者に対し,コイル塞栓を施行した.しかし,術11時間後から左頸部の腫脹がさらに増大し,CTA上左頸部内頸動脈の紡錘状動脈瘤が増大していたため,動脈瘤の末梢側および中枢側内頸動脈をコイル塞栓し,動脈瘤のトラッピングを行った.術後虚血合併症はなく経過したが,術1カ月半後に左頸部の皮下膿瘍,皮膚の自壊を認め,血腫除去および皮下郭清を施行した.その後の経過は良好であった.本症例は,頸部内頸動脈の血栓化動脈瘤が急速に増大して顔面動脈を物理的に損傷して皮下出血を来し,出血源である顔面動脈の処置後も動脈瘤が増大し続けたと思われた.
