フッ化ピリミジン系抗癌剤(以下5-FU)は体内でdihydoropyrimidine dehydrogenase(DPD)によって代謝され,DPD欠損や活性低下例では重篤な副作用を発症する.今回,CapeOX投与後に重篤な副作用を呈して死亡した症例を経験した.症例は58歳の男性.先天性小児麻痺を罹患,PS0.主訴は全身倦怠,食欲不振.現病歴はDay7までcapecitabineを内服後,摂食不良と脱水で入院.入院時Grade3口内炎と下痢,好中球減少を認めcapecitabine最終内服9日目から発熱性好中球減少症とGrade3口内炎を認めた.Nadir後状態改善し53日目に経口開始したが発熱,頻脈,意識レベル低下し,突然心停止となり死亡された.末梢血単核球DPD活性値は4.47U/mgと低値を示し,DPD活性低下症と診断された.5-FU投与時は副作用に注意し,本症例の早期発見に努める必要がある.
