The immunoglobulin receptor Fc gamma RIIIa (CD16) is distributed on natural killer (NK) cells, macrophages, and delta T polymorphic. Variation screening for the immunoglobulin G Fc receptor IIB (Fc(gamma)RIIB) gene was performed with genomic DNA from 100 healthy Japanese subjects. We identified 3 non-synonymous and 2 synonymous substitutions single-nucleotide polymorphisms in an intron region. Intraductal tubulopapillary neoplasm (ITPN) is a rare disease accounting for approximately 3% of all intraductal pancreatic tumors, with papillary mucinous (IPMN) being one the most common differential diagnoses. 頚部血栓化巨大動脈瘤に対し,後頚三角を利用して動脈瘤切除を行った症例を経験したので報告する.症例は82歳女性.最大径24.1mmの左頚部動脈瘤を認めるも,無症状のため経過観察していた.6年後に一過性右上下肢麻痺が出現し,MRI拡散強調画像では左前頭葉皮質に新鮮脳梗塞を認め,左頚部動脈瘤は最大径72.2mmに拡大していた.手術では乳様突起から胸鎖乳突筋前縁に沿った皮膚切開に加え,その下端から鎖骨上を外側に延長させたL字切開を行った.動脈瘤外壁には内頚静脈や周囲組織が強く癒着していた.内シャント設置後,動脈を切開した.動脈瘤内は器質化した血栓が充満しており,潰瘍形成している部分もあった.周囲組織との剝離後,動脈瘤を切断し,正常な血管壁同士を縫合して手術終了した.頚部動脈瘤に対する手術では,適切な術式選択もさることながら,適切な術野展開も重要である.
