The safety of drug-eluting stents has been called into question by recent reports increased stent thrombosis, myocardial infarction, and death. Such studies have inconclusive because their insufficient size, the use historical controls, a limited duration follow-up, lack access to original source data.We performed pooled analysis data from four double-blind trials in which 1748 patients were randomly assigned receive either sirolimus-eluting or bare-metal five 3513 paclitaxel-eluting stents;...

Background— Inhibition of the acyl coenzyme A:cholesterol acyltransferase (ACAT) enzyme may prevent excess accumulation cholesteryl esters in macrophages. The ACAT inhibitor avasimibe was shown to reduce experimental atherosclerosis. This study designed investigate effects on human coronary Methods and Results— randomized, double-blind, placebo-controlled trial assessed at dosages 50, 250, 750 mg QD progression atherosclerosis as by intravascular ultrasound (IVUS). All patients received...

We investigated the comparability of platelet function tests in quantifying inhibition achieved by clopidogrel. This pre-specified substudy a randomized, double-blind trial included 116 patients with stable coronary artery disease requiring diagnostic angiography. Patients received clopidogrel for 1 (300 or 600 mg) 7 days + 75 150 mg daily) before procedure. Blood samples obtained initiation and angiography were assayed using light transmission aggregometry [adenosine diphosphate (ADP) 5 20...

Background: Complete revascularization with routine percutaneous coronary intervention of nonculprit lesions after primary improves outcomes in ST-segment–elevation myocardial infarction. Whether this benefit is associated lesion vulnerability unknown. Methods: In a prospective substudy the COMPLETEs trial (Complete vs Culprit-Only Revascularization to Treat Multi-Vessel Disease After Early PCI for STEMI), we performed optical coherence tomography at least 2 arteries before 93 patients...

Objectives The objective was to assess the effect of ultrasound (US)‐guidance compared anatomical landmark (AL) approach in patients requiring femoral artery (FA) access for coronary angiography/percutaneous interventions (PCI). Background US‐guidance has been proposed as a strategy optimize FA access, potentially leading decreased vascular complications. Methods Patients angiography/PCI were randomized US‐guided or AL approaches. primary endpoint composite immediate procedural outcomes, and...

Importance In patients with multivessel coronary artery disease (CAD) presenting ST-segment elevation myocardial infarction (STEMI), complete revascularization reduces major cardiovascular events compared culprit lesion–only percutaneous intervention (PCI). Whether also improves angina-related health status is unknown. Objective To determine whether angina in STEMI and CAD. Intracoronary stenting as a transcatheter treatment for bifurcation stenoses remains controversial. A novel technique of coronary is reported. 15 mm Palmaz-Schatz stent bent 180° at its bridge articulation into V- configuration and mounted on two balloon-catheters linked together by adhesive tape. This unified delivery system was used successfully in five cases porcine stenting. © 1996 Wiley-Liss, Inc.

Activation of myocardial A2A adenosine receptors during reperfusion has been shown to be cardioprotective. The intracellular mechanisms underlying this protection remain unknown. To understand the beneficial effects activated in such a state, we investigated whether enzymes phosphatidylinositol 3-kinase (PI3K) and caspase-3 can account for post-ischemic cardioprotective effect an anesthetized rabbit model infarction (30 minutes ischemia; 5 hours reperfusion).
