背景.肺原発多形癌の臨床像とその治療法を手術症例で検討する.対象・方法.2016年1月から2021年6月の間に当院で手術を施行した肺原発多形癌7例の臨床像を後方視的に検討した.結果.対象7例は男性5例と女性2例で,年齢は64~81歳であった.臨床病期はIA期3例,IB期1例,IIA期1例,IIB期1例,IIIA期1例で,術式は肺全摘1例,肺葉切除5例,区域切除1例であった.術後無再発の2例は31ヶ月,28ヶ月間生存中であるが,5例に術後再発が認められ,うち2例は術後早期に癌死した.他3例には副腎転移が見られ,両側副腎転移の1例には化学療法と免疫チェックポイント阻害薬を投与し,術後32ヶ月も無再発生存中である.片側副腎転移の2例には副腎摘除術を施行,1例には化学療法を追加し,66ヶ月と52ヶ月の間無再発生存中である.結語.肺原発多形癌術後の単発の片側副腎転移症例は転移巣切除や免疫チェックポイント阻害薬投与により予後良好であった.一部の症例では転移巣を含む積極的な外科的治療の介入が長期生存の一助となり得る可能性が示唆された.
