症例は66歳，男性．中咽頭癌（cT1N0M0，cStage I）のスクリーニング検査で胸部中部食道に1型，2cmの食道扁平上皮癌（cT2N1M0，cStage II）と，肝S4，5，8にわたる8cmの腫瘍を発見．手術侵襲度を考慮して，一期目に経口的中咽頭部分切除術，二期目に胸腔鏡下胸部食道切除亜全摘術と一時的頸部食道瘻造設術，三期目に腹腔鏡下肝中央二区域切除術，用手補助腹腔鏡下胃管再建術を行う方針とした．二期目では食道口側を胸部上部食道レベルで切離し一時的食道瘻を造設，噴門側は胸部下部食道レベルで仮切離し盲端とした．二期目から4週後の三期目では腹腔鏡下肝中央二区域切除術に続き用手補助腹腔鏡下胃管再建術を開始，食道盲端部が縦隔内で高度に癒着していたため開腹手術への移行を要したが，三期分割切除にすることで一連の手術治療を完遂した．ただし，鏡視下で手術を完遂するためには，インターバル期間の再考や食道切離端処理の工夫が必要と考えられた．
