Data for 479 patients were analyzed to assess the impact of methicillin resistance on outcomes with Staphylococcus aureus surgical site infections (SSIs). Patients infected methicillin-resistant S. (MRSA) had a greater 90-day mortality rate than did methicillin-susceptible (MSSA; adjusted odds ratio, 3.4; 95% confidence interval, 1.5-7.2). MRSA duration hospitalization after infection (median additional days, 5; P<.001), although this was not significant multivariate analysis (P=.11). Median...
To evaluate the impact of methicillin resistance in Staphylococcus aureus on mortality, length hospitalization, and hospital charges.A cohort study patients admitted to between July 1, 1997, June 2000, who had clinically significant S. bloodstream infections.A 630-bed, urban, tertiary-care teaching Boston, Massachusetts.Three hundred forty-eight with bacteremia were studied; 96 methicillin-resistant (MRSA). Patients methicillin-susceptible (MSSA) MRSA similar regarding gender, percentage...
The polymyxin antibiotics colistin (polymyxin E) and B became available in the 1950s thus did not undergo contemporary drug development procedures. Their clinical use has recently resurged, assuming an important role as salvage therapy for otherwise untreatable gram-negative infections. Since their reintroduction into clinic, significant confusion remains due to existence of several different conventions used describe doses polymyxins, differences formulations, outdated product information,...
The efficacy of ceftazidime-avibactam—a cephalosporin–β-lactamase inhibitor combination with in vitro activity against Klebsiella pneumoniae carbapenemase–producing carbapenem-resistant Enterobacteriaceae (CRE)—compared colistin remains unknown. Patients initially treated either ceftazidime-avibactam or for CRE infections were selected from the Consortium on Resistance Against Carbapenems and other (CRACKLE), a prospective, multicenter, observational study. Efficacy, safety, benefit-risk...

We studied outcomes of extended-spectrum beta-lactamase (ESBL) production in Enterobacteriaceae bacteremia. Inpatients with bacteremia caused by ESBL-producing Escherichia coli, Klebsiella spp., or Proteus spp. (cases) were compared patients non-ESBL producers (controls). Outcomes included mortality, mortality due to infection, length stay (LOS), delay appropriate therapy (DAT), discharge a chronic care facility, and hospital cost. Ninety-nine cases 99 controls enrolled. Thirty-five percent...
Treatment options for carbapenem-resistant Enterobacteriaceae (CRE) infections are limited and CRE remain associated with high clinical failure mortality rates, particularly in vulnerable patient populations. A Phase 3, multinational, open-label, randomized controlled trial (TANGO II) was conducted from 2014 to 2017 evaluate the efficacy/safety of meropenem-vaborbactam monotherapy versus best available therapy (BAT) CRE.A total 77 patients confirmed/suspected infection (bacteremia,...

The β-lactamase inhibitor relebactam can restore imipenem activity against imipenem-nonsusceptible gram-negative pathogens. We evaluated imipenem/relebactam for treating infections.Randomized, controlled, double-blind, phase 3 trial. Hospitalized patients with hospital-acquired/ventilator-associated pneumonia, complicated intraabdominal infection, or urinary tract infection caused by (but colistin- and imipenem/relebactam-susceptible) pathogens were randomized 2:1 to 5-21 days...
Background. Colistin, originally abandoned due to high rates of nephrotoxicity, has been recently reintroduced activity against carbapenem-resistant Gram-negative organisms. Recent literature, largely obtained from outside the United States, suggests a lower rate nephrotoxicity than historically reported.
Preventable healthcare-associated infections (HAIs) occur in US hospitals. Preventing these is a national priority, with initiatives led by healthcare organizations, professional associations, government and accrediting agencies, legislators, regulators, payers, consumer advocacy groups. To assist acute care hospitals focusing prioritizing efforts to implement evidence-based practices for prevention of HAIs, the Society Healthcare Epidemiology America Infectious Diseases Standards Practice...

Importance
Meropenem-vaborbactam is a combination carbapenem/beta-lactamase inhibitor and potential treatment for severe drug-resistant gram-negative infections.
Objective
To evaluate efficacy adverse events of meropenem-vaborbactam in complicated urinary tract infection (UTI), including acute pyelonephritis.
Design, Setting, Participants
Phase 3, multicenter, multinational, randomized clinical trial (TANGO I) conducted November 2014 to April 2016 enrolling...
ABSTRACT The emergence of multidrug-resistant (MDR) Klebsiella pneumoniae has resulted in a more frequent reliance on treatment using colistin. However, resistance to colistin (Col r ) is increasingly reported from clinical settings. genetic mechanisms that lead Col K. are not fully characterized. Using combination genome sequencing and transcriptional profiling by RNA (RNA-Seq) analysis, distinct were found among nine isolates. was related mutations three different genes strains, with...
