症例は65歳男性.C型肝炎に伴う肝細胞癌および右腎細胞癌に対し一期的に外科的切除術施行.その後,経過観察中にみつかった肝細胞癌の孤立性再発に対しラジオ波焼灼術にて治療するも,糖尿病に伴う慢性腎不全が徐々に進行し,透析導入を考慮するまで悪化した(血清クレアチニン値;5.39).このころ,MRI検査で多発性肝癌の再発が明らかとなり透析導入は見送られ,血管造影下の治療も施行不可能となった.肝予備能がChild Aであったためソラフェニブを400 mg/日で投与開始した(腫瘍マーカー:AFP 3774 ng/ml,PIVKA-II 14995 mAU/ml).投与後1週間前後にgrade 3の水様性下痢が出現し,200 mg/日に減量とし症状が安定したので外来フォローとなった.腫瘍マーカーは投与5カ月後には正常化し,MRI検査では多発する腫瘤像は縮小しPRと判断された.また造影エコーでも腫瘍部の血流は認めず効果は良好と判断された.特記すべきことはソラフェニブ投与後,血清クレアチニン値は徐々に低下し2013年3月現在,1.45まで改善し腫瘍マーカー正常で継続投与中である.
