Group IIA secreted phospholipase A(2) (sPLA2) is known to display potent Gram-positive bactericidal activity in vitro and vivo. We have analyzed the of full set recombinant murine human groups I, II, V, X, XII sPLA2s on Listeria monocytogenes, Staphylococcus aureus, Escherichia coli. The rank order potency among against bacteria group > X V IIE IB, IIF (for sPLA2s: IID IIC, IIF), only displays detectable Gram-negative bacterium E. These studies show that highly basic with exception ability...
Background. The definition of a "difficult" cannulation varies considerably in reports endoscopic retrograde cholangiopancreatography (ERCP). Aims. To define difficult cannulation, which translates into higher risk post-ERCP pancreatitis. Patients and methods. Prospective consecutive recording 907 cannulations Scandinavian centers done by experienced endoscopists. Inclusion: indication for biliary access patients with intact papilla. Exclusion: acute non-biliary chronic pancreatitis at time...
Abstract Objectives When bloodstream infections are caused by resistant bacteria, rapid antimicrobial susceptibility testing (RAST) is important for adjustment of therapy. The EUCAST RAST method, directly from positive blood cultures, was validated in a multi-laboratory study Europe. Methods performed 40 laboratories northern Europe (NE) and 15 southern (SE) clinical cultures Escherichia coli, Klebsiella pneumoniae, Pseudomonas aeruginosa, Staphylococcus aureus or Streptococcus pneumoniae....
Non-operative management of uncomplicated acute appendicitis is an option, but omission antibiotics from the regimen has not been tested.A double-blind, placebo-controlled, superiority RCT in adults with CT-confirmed was designed to compare placebo (intravenous ertapenem followed by oral levofloxacin and metronidazole). The primary endpoint treatment success (resolution resulting discharge without appendicectomy within 10 days); secondary outcomes included pain scores, complications,...
Group IIA phospholipase A2 (PLA2-IIA) is a newly recognized antibacterial acute phase protein. The concentration of PLA2-IIA increases up to 500-fold in the blood plasma patients with severe diseases, compared healthy persons. Despite numerous studies, exact roles this enzyme human diseases are unknown. This study investigated properties serum. serum samples bacterial infections was capable killing 90% Staphylococcus aureus and 99% Listeria monocytogenes vitro after incubation for 2 h. At...
Abstract Group V phospholipase A2 (PLA2) is a recently characterized 14-kDa secretory PLA2 of mammalian heart and macrophage-derived cells. IIA PLA2, which structurally close to group has been shown kill Gram-positive bacteria in vitro prevent symptoms infection vivo. We studied the antibacterial properties fully active recombinant rat PLA2s. Both PLA2s were highly bactericidal against bacteria, including methicillin-resistant staphylococci vancomycin-resistant enterococci. Only high...
We report two cases of louse-borne relapsing fever (LBRF) in young Somali asylum seekers having recently arrived to Finland. They had sought medical attention for a febrile illness. Blood smears were examined suspected malaria, but instead, spirochete shaped bacteria observed. The confirmed as Borrelia recurrentis by PCR and sequencing. patients survived, their treatment was complicated Jarisch-Herxheimer reaction. conclude that LBRF must be considered diagnostic option refugees also the...
Abstract Carbapenemase-producing Enterobacterales (CPE) pose an increasing threat to patient safety and healthcare systems globally. We present molecular epidemiology of CPE in Finland during 2012–2018 with detailed characteristics strains causing clusters the same time period. All Finnish clinical microbiology laboratories send isolates reduced susceptibility carbapenems or producing carbapenemase reference laboratory for further characterization by whole genome sequencing (WGS). In total,...
Abstract The complement system is regarded as an important component of the innate defence against invading bacteria. However, synergistic actions between and other components immunity are incompletely known. Human group IIA phospholipase A 2 (hGIIA PLA ) effective antibacterial enzyme in serum patients with severe bacterial infections. Our aim was to investigate significance hGIIA acute phase serum. Serum samples were collected from infections healthy control subjects. We prepared ‐depleted...
Cannulation of the papilla vateri represents an enigmatic first step in endoscopic retrograde cholangiopancreaticography (ERCP). In light falling numbers (diagnostic) ERCP and novel techniques, e.g. short-wire system, we were interested approach novice experienced endoscopist are taking; especially, what makes a difficult to cannulate how this. We devised structured online questionnaire, sent all endoscopists registered with SADE, Scandinavian Association for Digestive Endoscopy. A total 141...
Group IIA phospholipase A2 (PLA2-IIA) is an enzyme which has important roles in inflammation and infection. Recently, a novel human secretory PLA2 called group XIIA (PLA2-XIIA) been identified. Both PLA2-IIA PLA2-XIIA are bactericidal against Gram-positive bacteria like many other PLA2s. However, the only known displaying significant activity Gram-negative bacterium Escherichia coli. We examined antibacterial properties of recombinant Helicobacter pylori, bacterium, vitro. was not H. whereas...
Carbapenemase-producing Enterobacterales (CPE) have spread widely into health care facilities (HCF) but clusters caused by carbapenemase-producing (CP) Citrobacter freundii been uncommon until recent years. Here we describe CP C. detected in Finland during 2016-20.As a part of the national CPE surveillance, clinical microbiology laboratories send potential isolates to reference laboratory for confirmation and further characterization. Whole genome sequencing (WGS) with Illumina MiSeq...
Our objective was to assess the incidence of bacteraemic Aerococcus urinae cases at Helsinki metropolitan area, Finland, from a 6-year study period (2013 2018) and further characterize available cases. The evaluates outcome commonly used cefuroxime treatment determinate set A. in vitro antimicrobial susceptibilities for benzylpenicillin, cefuroxime, ceftriaxone. Clinical records patients were reviewed retrospectively. Antimicrobial susceptibility testing performed by disk diffusion, gradient...
We report an outbreak of invasive pneumococcal disease and pneumonia among shipyard workers, in Turku, Southwest Finland. In total, 31 confirmed six probable cases were identified between 3 May 28 November 2019. Streptococcus pneumoniae serotypes 12F, 4 8 isolated from blood cultures 25 cases. Occupational hygiene measures vaccination ca 4,000 workers are underway to control the at shipyard.
This study reports a previously healthy, immunocompetent adult male in whom human herpesvirus 7 (HHV-7) DNA was detected continuously from the cerebrospinal fluid (CSF). patient developed definite sarcoidosis with primary symptomatic manifestations central nervous system (CNS). The initial presentation loss of visual acuity and papilledema. Brain MR imaging at confirmed papilledema, but otherwise there were no focal abnormalities or signs hydrocephalus. CSF investigation revealed pleocytosis...
We describe a rare case of prosthetic valve endocarditis caused by the canine bacterium Capnocytophaga canimorsus in male aged 73 years. The diagnosis infective was unequivocal, as it blood cultures were positive for C. and vegetations detected on transesophageal echocardiography; modified Duke criteria fulfilled. PET ‐ CT showed intense 18 F‐ FDG uptake area. patient treated with antibiotics alone (no surgery), is now life‐long suppressive antibiotic therapy. To our knowledge, this third...
Abstract Aim This study examined the predisposing factors, clinical picture, bacterial aetiology and outcomes of infants children with meningitis (BM). Methods The medical records patients under 16 years age, treated by Turku University Hospital, Finland, from 2011 to 2018, were screened for using International Classification Diseases, Tenth Revision codes. Patients included if bacteria detected in cerebrospinal fluid (CSF) or other predefined laboratory variables indicated BM, despite CSF...
