BackgroundManipulation under anaesthesia and arthroscopic capsular release are costly invasive treatments for frozen shoulder, but their effectiveness remains uncertain. We compared these two surgical interventions with early structured physiotherapy plus steroid injection.MethodsIn this multicentre, pragmatic, three-arm, superiority randomised trial, patients referred to secondary care treatment of primary shoulder were recruited from 35 hospital sites in the UK. Participants adults (≥18...
With the proliferation of different fixation screws, there is an increasing trend to recommend early internal broken scaphoid even if fracture not displaced. The benefits and risks fractures have been established. These were investigated in eighty-eight patients who working age with clearly defined minimally displaced or undisplaced bicortical waist scaphoid.Patients provided informed consent randomized treatment use a Herbert screw without cast (forty-four patients) nonoperative for eight...
Pedicled vascularized bone grafts (Zaidemberg's technique) were used to treat 22 established scaphoid fracture nonunions, 16 of which found have avascular proximal poles at surgery. After a follow-up 1-3 years, only six (27%) the nonunions had united. Only two with united, compared four vascular poles. We conclude that this technique pedicled grafting may not improve union rate for pole fragments.
This article examines how financial inducements in performance contracts shape the inner workings of a for-profit welfare-to-work training program serving long-term recipients. Our work pays particular attention to contract requirements relationships between manager and line staff their treatment clients. We argue that design, coupled with bottom-level management efforts meet contractual obligations, leads paradox—the same actions taken achieve results ironically produce negative practice...
Purpose To examine the impact of a personal health record (PHR) in patients with hypertension measured by changes biological outcomes, patient empowerment, perception quality care, and use medical services.
This study explores the epidemiology of patients with a fracture scaphoid presenting to regional teaching hospital. All confirmed over retrospective period between January 2010 and May 2013 were included. Their demographics, deprivation status when occurred was noted assessed. There 415 fractures in 365 males 50 females. The incidence 12.4 100 000 each year general population. The mean age 22 years (nine 35); highest aged 15 19 years. We calculate annual United Kingdom be 7265 year. Patients...
This multi-centre postal questionnaire study was conducted by the Audit Committee of BSSH to assess outcomes surgery for Dupuytren’s Contracture using subjective data provided 1177 patients at a mean follow-up 27 (SD 8) months after surgery. Surgery contracture achieved high rate full, or almost correction in 826 (75%) but had incidence post-operative patient-reported complications 46%. A higher complication seen those with worse initial deformities. The recurrence persistence 158 1037...
The purpose of this prospective clinical study was to identify the true incidence, pattern, and location injury nature fracture after hand injuries in different pediatric age groups attending a unit. Three hundred sixty children (237 boys, 123 girls) under 16 years who presented with between April 1, 2000, Sept. 30, were included study. Bony accounted for 65.5% (236 injuries); 33.3% (120 injuries) soft tissue injuries. projected annual incidence rate skeletal 418/100,000 children. low...
The different attributes of the Patient Evaluation Measure (PEM) questionnaire were investigated in 80 patients with a fracture scaphoid. Assessments made at 2, 8, 12, 26 and 52 weeks. Reliability was assessed by measurement internal consistency questions 275 completed PEM forms. Cronbach's alpha, which needs to lie between 0.7 0.9, 0.9 for PEM. Pain, tenderness, swelling, wrist movement grip strength correlated score confirming validity assessment. Changes variables visits significantly...
The Patient Evaluation Measure (PEM), Michigan Hand Outcome Questionnaire (MHQ) and the Disabilities of Arm, Shoulder (DASH) score were assessed independent their originators for reliability, construct criterion validity acceptability, using an ease use questionnaire. These administered in random order to 100 patients with different hand wrist disorders impairments movement, pain, sensation strength. internal consistency all three questionnaires was very high suggesting redundancy questions....
We have evaluated the long-term outcome of excision, aspiration and no treatment dorsal wrist ganglia prospectively in 236 (83%) 283 patients who responded to a postal questionnaire at mean 70 months. The resolution symptoms was similar between groups (p>0.3). Pain unsightliness improved all three groups. prevalence weakness stiffness altered only slightly More with recurrent, or persistent ganglion complained pain, (p<0.0001). Patient satisfaction higher after surgical excision (p<0.0001),...
Age related differences in demographics, morphology, treatment and outcome were investigated 701 fractures of the metacarpals or phalanges, including fracture-dislocations, 655 patients. Fractures mainly due to sport occurred 184 children, usually after 10 years age. The base proximal phalanx was especially vulnerable. Thirty-seven percent 256 young adults fractured their fifth metacarpal. thumb rarely involved. Half these two groups ray. Older had more distal displaced extraarticular...
We randomised 79 patients (84 hands, 90 fingers) with Dupuytren’s contracture of the proximal interphalangeal joint to have either a ‘firebreak’ skin graft (39 patients, 41 44 or fasciectomy (40 43 46 if, after full correction, over phalanx could be easily closed by Z-plasty. Patients were reviewed three, six, 12, 24 and 36 months note any complications, range movement recurrence. Both groups similar in regard age, gender factors considered influence outcome such as bilateral disease, family...
Distal radial fractures are the most common adult orthopaedic fracture. We sought to determine whether incidence of this injury is changing and identify trends in its occurrence. analysed data for all patients presenting University Hospitals Leicester with a distal fracture from 2007–2016. Incidence rates were calculated using United Kingdom population data. Poisson regression techniques used analyse weekly, seasonal annual variation incidence. There was no significant change average age or...
Treatments for Dupuytren's contracture include limited fasciectomy and collagenase injection. Comparisons of the effectiveness these treatments have been limited.
The sport of rock climbing has its own spectrum injuries, almost half which involve the wrist and hand.To examine incidence acute hand injuries in 545 members Climbers' Club Great Britain.A total 1100 questionnaires were sent to current Britain for them detail any sustained date. In decade years, grades time spent at each grade determined. From these results a historic intensity score climber could be calculated.Half returned complete. respondents entirely male. There 235 155 climbers (28%)....
We describe a new method of stabilising painful unstable sternoclavicular joint using the sternocleidomastoid tendon and passing it through medial clavicle onto manubrium sternum. This is simple, reproducible avoids potential risks reefing to first rib. The technique was used in seven cases instability six patients who were reviewed at mean 39.7 months (15 63). Instability markedly reduced or eliminated all cases, but one there occasional persistant subluxation. There minor scar...
We report the outcome at a mean of 93 months (73 to 110) 71 patients with an acute fracture scaphoid who were randomised Herbert screw fixation (35) or below-elbow plaster cast immobilisation (36). These represent majority series 88 whose short-term has previously been reported. Those available for later review similar in age, gender and hand dominance. There was no statistical difference symptoms disability as assessed by Patient Evaluation Measure (p = 0.4), Patient-Rated Wrist 0.9), range...
