症例は63歳の女性で,検診で腹部超音波検査を受けた際に,膵腫瘍を指摘された.CTで膵尾部腹側に33 mmの低吸収腫瘤を認めた.病変は漸増性にやや不均一に造影された.MRIでは,T1強調像で低信号,T2強調像では中心部が低信号,辺縁部が淡い高信号,拡散強調像では辺縁部で高信号,中心部で低信号を呈した.以上の所見から,solid pseudopapillary neoplasm(SPN)やデスモイド腫瘍が疑われたが確定診断には至らず,診断的治療目的に手術の方針とした.術中所見では,腫瘍は膵尾部に存在し,胃大彎側の胃壁に浸潤していた.悪性腫瘍も疑われたため,腹腔鏡下脾合併膵体尾部切除,胃部分切除を施行した.術後病理組織診でデスモイド腫瘍と診断された.腹腔鏡下膵切除を施行したデスモイド腫瘍の報告はまれであり報告する.
