手術侵襲に対する過大な生体反応の制御を目的に, 食道癌手術症例に対し手術開始2～3時間前にmethylprednisolone 250mg/bodyを投与 (B群: n=8) し, コントロール群 (A群: n=9) と比較検討した. 心係数・平均肺動脈圧・肺動脈楔入圧の変動に差はなかったが, 体重・水分1次出納はB群で1日早く回復した. 術後頻拍はB群で有意に制御され, 不整脈の発生もB群で少ない傾向であった. A群で術後著増したIL-6やG-CSFはB群では有意に産生が抑制され, 尿中カテコールアミン排泄量もB群で著減した. 耐糖能異常・術後感染・縫合不全などステ古ィド剤の副作用は術後みられず, 術後のリンパ球数減少もB群で抑えられた. したがって, 術前ステロイド投与は食道癌手術侵襲に対する過大な生体反応を主に循環系の面で抑制し, 不整脈などの合併症発生を減少させうることが示唆され, 胸部食道癌手術後の患者管理において非常に有用と考えられた.
症例は84歳の男性で, 不明熱として発症して約2カ月後に消化管穿孔による腹膜炎のため緊急開腹手術となった. 小腸未分化癌の壊死穿孔による腹膜炎で, 広範囲にリンパ節転移を認めた. 術後25日で退院したが, 術後39日目に発熱を伴う癌性腹膜炎の状態で再入院し, 術後45日目に死亡した. 小腸未分化癌は病理組織学的にも診断は困難であり, 悪性リンパ腫やgastrointestinal stromal tumor, 平滑筋肉種などの間葉系腫瘍と鑑別する必要がある. 今回の症例は免疫染色によりCK-CAM5.2, vimentinが陽性であったことから未分化癌と診断しえた. 小腸未分化癌は症状が多彩で早期診断が困難と思われる. 本症例の経過からも不明熱は初期症状の一つとして念頭におく必要があると思われた.

血液透析 (HD) 患者の低Alb血症に対する舌訓練の有効性を検討した. 舌圧が健常人の平均値と比較して100%未満かつAlb 3.8g/dL未満のHD患者を対象に, トレーニング器具 (ペコぱんだ®) を用いて5回3セット/回を1日3回, 週3回以上の舌訓練を指示して2か月間追跡した. 18名中, 指示を遵守できたのは12名 (男性8名, 女性4名, 72.3±10.5歳) であった. 訓練開始前後, 舌圧 (前: 22.5±6.9kPa vs. 後: 33.1±6.8kPa, p<0.01), Alb (3.58±0.20g/dL 3.73±0.29g/dL, p<0.05) に有意な差を認めた. また, 舌圧変化量とCRP変化量において有意な負の相関 (r=−0.581, を認めた. 本研究では, 炎症反応の低下とともにAlbと舌圧の上昇を認めた.
