坐骨ヘルニアは,坐骨孔より脱出するヘルニアであり,閉鎖孔ヘルニアとともに骨盤底ヘルニアに分類されている.今回われわれは,閉鎖孔ヘルニアに併存していた坐骨ヘルニアの1例を経験した.症例は93歳,女性.右臀部から大腿背側にかけての疼痛,腹部膨満,嘔吐を主訴に受診した.腹部CT検査にて,右閉鎖孔ヘルニア嵌頓とそれに伴う小腸イレウス,右坐骨ヘルニアと診断し,手術を施行した.右閉鎖孔に小腸が嵌頓し壊死していたため,小腸部分切除術を施行した.右坐骨孔の小腸の嵌入は開腹時に自然整復されており,ヘルニア嚢の奥に傍卵巣嚢腫の嵌入を認め嚢腫切除術を施行した.右閉鎖孔・右坐骨孔は単純閉鎖した.閉鎖孔ヘルニアと坐骨ヘルニアは,骨盤底筋群の脆弱化という同様の機序がある.坐骨ヘルニアはすべてのヘルニアの中で最も稀と報告されているが,無症候性の場合もあり,両疾患の合併を念頭に置いた診断・治療が必要であると考えられた.
