症例は65歳男性,軽症の急性膵炎の診断で入院となった.造影CT,MRIで膵尾部の主膵管および分枝膵管の拡張を認めた.超音波内視鏡にて主膵管狭窄部の近傍に分枝膵管の拡張と思われる無エコー領域とそれと連続する約1cmの腫瘤とは認識できない低エコー領域を認めた.膵管造影で主膵管の狭窄部を認め,連続膵液細胞診にて異型細胞を認めた.以上の結果より,膵管癌を疑い,手術を施行した.病理組織で主膵管狭窄部近傍の分枝膵管に上皮内癌の存在が明らかとなり,膵上皮内癌の診断に至った.低エコー領域は膵上皮内癌の間接的な画像所見であると考えられ,同様の所見を認めた場合は,連続膵液細胞診を行うことで,膵上皮内癌を診断することができると推察された.
本邦における有棘細胞癌およびボーエン病の現状を把握するため，全国調査を行った。全国約180の主要施設に対して，調査用紙を送付し回収した。2008年1月から12月までの1年間に各施設を受診した新規症例を対象とし，有棘細胞癌1050例，ボーエン病741例の有効回答を得た。有棘細胞癌の年齢分布は29歳から106歳で平均が77.8歳となった。発生部位は頭頸部536例（51.0％），体幹101例（9.6％），上肢92例（8.7％），下肢178例（16.9％）であり，頭頸部の発生が多い。熱傷瘢痕が発生母地の場合は病期IVの割合が多い傾向があった。ボーエン病の年齢分布は32歳から102歳で平均が75.2歳となった。単発例が678例，多発例が59例であった。発生部位は下腿が最も多く154例（20.7％）で，大腿が74例（9.9％），背部が67例（9.0％）の順となった。有棘細胞癌の所属リンパ節転移のリスク因子解析では，頭頸部を基準とした場合，外陰部・体幹・下肢が高リスクとなった。

肝門部胆管癌と診断し肝外胆管切除を伴う肝葉切除を施行した良性胆道狭窄の2例を報告する．いずれの症例も黄疸を主訴に他院にて胆道ドレナージを受けた後，来院した．右肝管を中心に著明な狭窄像を呈し，CTでは腹腔内リンパ節腫大が目立った．膵臓には異常所見を診断しえなかった．また，胆管像では進行性硬化性胆管炎に特徴的な所見を認めず臨床症状，画像検査所見からは良性疾患と診断しえなかった．いずれの症例も肝右葉切除，尾状葉切除，肝外胆管切除術を施行した．病理組織学的検討ではいずれも粘膜下に著明なlymphoplasmacytic inflammatory infiltrationを認める慢性胆管炎の所見でIgG4免疫染色検査は陰性であった．肝門部胆管狭窄病変を診断する際には，本報告で示したような症例があることを常に念頭におき，慎重な対応を心がける必要がある．

肝原発扁平上皮癌の1剖検例を経験したので報告する．症例は59歳男性．発熱，心窩部を主訴に受診した．肝胆道系酵素，炎症反応の上昇を認め，肝右葉から肝門部にかけて8 cmの腫瘤性病変を認め，総胆管を閉塞していた．緊急でERCPを施行し，胆道ドレナージを行った．総胆管閉塞部からの擦過細胞診と胆汁細胞診で扁平上皮癌を認めた．また，腫瘍は十二指腸と横行結腸へ浸潤しており，腫瘍結腸瘻を形成していた．以上より，肝内胆管癌，肝門部・十二指腸・横行結腸浸潤と診断した．腫瘍が広く浸潤しているため根治手術は困難であり，人工肛門造設，消化管バイパス，胆道ステント留置後に全身化学療法を施行した．治療効果は乏しく，受診から約4カ月で永眠された．剖検を施行し，腫瘍の成分はすべて扁平上皮癌であり，肝原発扁平上皮癌と診断した．
