症例は62歳の男性.2002年11月に検診で発見された胃癌に対し幽門側胃切除術を施行した.病理所見はtype0-IIc+IIa,37×27mm,por1,pT1b,INFa,ly3,v1,N2,pStage IIAであった.術後補助療法は施行せず,経過観察中であった.2003年2月より,ふらつき・頭痛が出現し精査施行したところ,右小脳に3.0cm大の転移性脳腫瘍を指摘.脳内の他部位に転移巣を認めず,肝肺局所に再発所見は認めなかった.右小脳転移性脳腫瘍に対し開頭腫瘍摘出術を施行.術後経過は良好であり,現在術後10年以上無再発生存している.胃癌からの脳転移は,全身に転移が認められた後の末期的な状態として発見される場合が多く,今回の症例のように孤立性に転移をきたすことは稀である.今回,われわれは胃癌術後に発症した孤立性小脳転移に対し加療により良好な予後を得られた1例を経験したので,文献的考察を加えて報告する.
