抗体は,現在では医薬品創出に欠かせないフォーマットとして確立し,幅広く認知されている.だが,一口に抗体医薬といっても,複数の要素技術が採用されており,複合的な技術の蓄積が今日の繁栄を支えている.開発初期には,マウスで取得した抗体の抗原性が問題となっていた.この課題を,マウスなどの異種の動物由来の抗体を部分的にヒト抗体配列に置き換える,キメラ抗体技術や,ヒト化抗体技術,さらには,ファージディスプレイ法やヒト抗体産生遺伝子改変マウスなどによりヒト抗体を取得する方法を開発することにより克服してきた.一方,ADCC活性などの抗体本来の機能をさらに高めるポテリジェント(Potelligent®)技術や,新規機能を付加したバイスペシフィック抗体技術などを搭載した抗体薬品の臨床試験が進んでおり,徐々に上市され始めている.本レビューでは,現在までの抗体技術開発の経緯と将来の技術開発の展望を解説する.

We recently encountered 5 cases of sarcoidosis accompanied with facial paralysis. The courses paralysis varied, and it presented diverse clinical profiles including bilateral paralysis, latent nervous disorder, unilateral difficult to distinguish from Bell's palsy. 眼球病変にガンマナイフ治療を施行する際,target pointに正確に照射するために眼球運動を完全に制御する必要がある.症例は54歳,男性.1992年8月,肺癌に対し中下葉切除および化学療法を施行した.2000年10月,左視力障害が出現し,MRIにて左脈絡膜転移および多発性脳転移を認め当院紹介となった.ガンマナイフ治療に際し,眼球は瞬目麻酔後,点眼麻酔を適宜使用しながら4直筋に制御糸をかけて完全に固定した.辺縁線量15Gyにて照射を行い,20週間後も視力の悪化は認められず,有効な視力を維持することができた.本治療法は,眼球転移病変に対し,その効果,低侵襲性から極めて有効な治療法であると考えられた.
