肺結核治療の人工気胸術や結核性胸膜炎後の慢性膿胸に合併する悪性リンパ腫, いわゆる膿胸関連悪性リンパ腫pyothorax-associated lymphomaに対する外科治療を経験したので報告する. 症例は71歳, 女性. 既往歴として19歳時, 肺結核に対し人工気胸術を施行されている. 慢性膿胸として他院にて経過観察中, 血痰, 喀血が出現し, 有瘻性膿胸と診断され当院へ紹介された. 開窓術後, 胸壁胸膜に悪性リンパ腫の小病巣が発見され, 放射線治療にて画像上の寛解が得られ, 胸膜肺全摘術を施行した. 切除肺および胸膜に悪性リンパ腫は認められなかった. 術後1年8ヵ月現在, 再発無く健在である.
