症例は70歳代女性．C型慢性肝炎（HCV genotype 1b，低ウイルス量）につき当科通院中であった．過去4回のインターフェロン（IFN）治療歴があるが，いずれも著効は得られず，Daclatasvir（DCV）・Asunaprevir（ASV）併用療法を開始した．同薬内服18日目に急性胆嚢炎の疑いで当科入院となった．入院時に血小板数7.3万/μlとやや低値であったが，胆嚢炎症状が改善した後も血小板数が1.1万/μlまで低下し，血小板輸血を要した．血小板減少の原因としてDCV・ASVの影響が否定できず，同薬の内服は22日間で中止とした．中止後1週間ほどで血小板数は徐々に増加し，中止後24日で退院，中止後24週間での持続ウイルス陰性化も確認できた．HCV陽性患者における血小板減少は，IFNベースの治療時のみならず，IFNフリーのDirect Acting Antiviralsによる抗ウイルス治療時にも注意を要する副作用の一つと考えられるため，文献的考察を加え報告する．
