目的: 总结腹腔镜下逆向鞘内游离保留直肠上动脉的左半结肠根治术临床效果。 方法: 采用描述性病例系列研究方法,回顾性分析2017年10月至2019年3月间海军军医大学第二附属医院肛肠外科完成的腹腔镜下逆向鞘内游离保留直肠上动脉的37例左半结肠癌根治术患者的临床资料。主要手术步骤:采用腹腔镜5孔法,打开乙状结肠后Toldt间隙,于直肠上动脉侧先行打开动脉鞘,向肠系膜下动脉根部方向逆向游离肠系膜下动脉鞘,显露左结肠动脉及乙状结肠动脉分支并结扎离断,保留直肠上动脉。观察患者术中及术后情况。 结果: 所有患者均顺利完成手术,其中36例患者成功保留直肠上动脉。手术时间为(163.1±24.7)min;保留直肠上动脉操作时间为(16.7±3.1)min;术中离断肠系膜下动脉分支数量为(2.7±0.8)支;保留直肠上动脉操作出血量为(6.5±3.4)ml;手术总出血量为(50.1±18.8)ml,术中均未损伤肠管;检取淋巴结数(13.7±4.3)枚。术后排气时间(2.7±0.6)d,恢复饮食时间(3.3±0.7)d,住院时间为(8.7±1.3)d;术后未出现吻合口漏等相关并发症。 结论:...
